Staffing and Training
Professionals are of variety needed in the building. Administrator, Director of Nursing, Department Heads and all of the direct care staff. They have to meet credentials for their position and then provide orientation, training on site in the building. Other training is provided as needed and any helpful information that helps them perform their jobs better.
Resident Care
All the TEAM is responsible for the care of the residents. Routine rounds being made, having 24/7 on call system in place where any additional support is needed. Main entry to the building is through the front door.
Support Services
Social Services, Admissions Director, Administrator, Dept Heads all play a role in this. Additionally, if any other services are needed, i.e. Psychiatric, ancillary such as dental, vision, etc. We make every effort to provide what each resident needs.
Transportation
We have a W/C accessible van. We are not usually the primary provider but do accommodate often when not available.
Dining Services
We have two dining rooms, served off steam tables and delivered to the residents- one for the Nursing Home side and one for the Assisted Living side. If a resident does not like the dining choice, alternatives are available.
Feedback and Complaints
Resident is encouraged to discuss with first caregiver they see. If this complaint is something that cannot immediately be rectified, supervisor notified- if cannot be immediately resolved, we do have a complaint form that is reviewed and make every attempt to close out the concern within 1-3 days.
Rates and Programs
Private pay rates room and board $255 private pay SNF and $155 daily rate in Assisted Living. We also have Assisted Living Medicaid Waiver program.
Resident and Family Communication
Initial care conference and then quarterly or whenever there is a change in condition or request from resident/family. Notify family when resident or family requests.
Accommodations
We offer both short term and respite stays in both our SNF and AL. Case by case basis. Working on this as do not have many open apartments or beds.
Vaccinations
Yes on the Covid and Flu vaccinations.
Age in Place
Absolutely age in place encouraged. Our environment is conducive to this since we have both Assisted Living and SNF in the same building.
