We have a LPN 5 days per week
We staff both CNA and Personal Care Aides
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
- We are a completely secure facility with exit doors being locked 24/7
- We are staffed 24/7
- We do individual care plans tailoring care to each resident's personal needs
Do you offer any resources or support services to aid residents' transition into the community?
We provide care to all of our residents encouraging them to attend activities and socialize with other residents

Could you tell me a little about your dining experience?
- We offer 3 meals and 3 snacks per day
- Menu changes daily and residents can ask for alternative dining choice if they don't like what the menu has to offer
What is the process that takes place if a resident has a complaint or an issue they want addressed?
Complaints are given to appropriate department head to investigate and discuss resolution with family or resident
What is your policy on communication with resident's families?
- Families can request meetings at anytime with staff
- Any resident changes in condition will always be followed up with a phone call
How do you keep them updated about their loved ones' well-being?
Care plan meetings are set up for every 6 months or with a significant change in condition to discuss residents current plan of care
