Memory Care | SERVICE LEVELS & PRICING
LEVEL ONE $4,050 per month — “Independent”Level One is for Residents who benefit from limited “cuing” to perform morning through bedtime routines, guidance to meals and activities, health and wellbeing monitoring, self-directed ADL’s (activities of daily living), stand-by assistance for bathing, and who have no incontinence issues, are low fall risks, require basic medication management, and exhibit minimal behaviors.
LEVEL TWO $5,500 per month — “Verbal Cuing”Level Two is designed to support the wellbeing of Residents who benefit from verbal cuing and assistance for ADL’s, stand-by or verbal cuing for showering, occasional assistance with incontinence, and verbal cuing and redirection for behavioral challenges. This level includes high-quality incontinence supplies plus wipes, creams, and undergarments.
LEVEL THREE $6,850 per month — “Hands-On”Level Three is designed to support the well-being of Residents who benefit from additional engagement and safety monitoring, require hands-on assistance for ADL’s, hands-on assistance for showering and bathing, frequent assistance with toileting and incontinence management, high-risk fall prevention, and mild to moderate hands-on behavioral support. This level includes high-quality incontinence supplies plus wipes, creams and undergarments.
LEVEL FOUR $8,050 per month — “2-Person/Intensive Cares”Level Four is designed to support the wellbeing of Residents who benefit from 2-person assistance for ADL’s, full assistance with bathing, showering, high-acuity medication and therapy management, 2-person transfers or mechanical lift services, hospice service management, and intensive behavioral management. This level includes high-quality incontinence supplies plus wipes, creams and undergarments.
MONTHLY SUITE RENTAL
Standard Suite — $4,050 per month (includes $400 meal plan)
Larger Suite — Additional $500 per month
CUSTOM CARE PACKAGES may be designed for individuals who have medical conditions requiring higher skilled health services management. These may include: ColostomyCare, Diabetes Management, Heart Disease Management / Oxygen Treatments, etc.
ONE-TIME RESERVATION COMMUNITY FEE — $3,500This fee supports a clinical in-home pre-assessment necessary to determine a Resident’s service level requirements and provides our team with an opportunity to explore history of origin, routines, habits and lifestyle as well as health records set-up, medical history review, pharmacy set-up, accounting systems enrollment, suite preparation, new Resident staff orientation, complementary moving services (our moving specialists will coordinate and execute moving and staging for all furnishings selected for your new Resident Suite), and family move-in meal. No security deposit required.
