We hope that you have enjoyed your tour and look forward to answering any questions that you may have. Enclosed please find our application which can be used for all 13 of our communities and a brochure that lists our services and amenities.
Current Daily Room Rates
- Private Room: $612.07 ($573.10 + 6.8% NYS tax assessment equaling $35.43)
- Semi-Private Room: $535.71 ($501.60 + 6.8% NYS tax assessment equaling $28.37)
Hospital Admissions
If your loved one is currently in the hospital, the hospital will provide us with any medical & financial information that we will need.
Admissions from Home Settings
If your loved one is coming from any other home setting, please see the below list of items that we will need to review your loved one's application:
- The enclosed application
- PRI & NYS PASSAR Screen (would need to be completed for you by a licensed home care agency)
- History and physical (from your primary medical doctor)
- Most current medical visit note (from your primary medical doctor)
- Any current lab work (if applicable from your primary medical doctor)
