ASSISTED LIVING

Rooms are priced on a monthly rental basis.
Your monthly fee provides access to all
common areas of your new home as well as
services and amenities that provide the staff
support needed to live a fulfilling life.

Community Fee: $3340
A one-time non-refundable community fee is required
upon lease signing

Basic Fee Schedule

The monthly fee includes:

• Your selected room
• Our dining room program including three well balanced,
  delicious meals daily plus snacks
• Activities and other specially designed programs and trips*
• Wellness Assessments every six months
• Weekly laundry and housekeeping services
• 24-hour emergency call system
• All utilities excluding private telephone and private cable
• Interior and exterior maintenance

*Expenses incurred by residents out on bus trips i.e., lunch, dinner, shopping, et
al are not included.

Medication Administration Fee

Medication
LEVEL 1-$325/month
Basic medication management-up to a total of (8) eight
medications
LEVEL 2-$495/month
More than (8) eight medications and/or diabetic management,
and/or injections, and/or nebulizer
treatments, and/or oxygen management, and/or daily vital
signs, and /or diagnostic lab management/testing.
If resident chooses not to use the facility provider pharmacy
of choice, a private use fee of $170.00/month will be applied.
This excludes Veterans Administration. In the event that the
private pharmacy, to include the Veterans Administration, does
not provide ordered medications in a timely manner, facility will
obtain the ordered medications from facility provider pharmacy
at the resident’s expense.

Monthly Room Rates:

Spousal Rate- $1,515/month-Room and Board
only, double occupancy required. Additional
ancillary fees and Care Packages will be applied.

RESPITE CARE

Respite Rates*
A minimum of seven (7) days is required.
No Community Fee
The following rates Includes all Levels of care, medication
management, laundry, housekeeping and cable.

• Assisted Living-$225-$425/day
• Memory Care-$250-$450/day

All potential respite stays must be pre-screened and
provide a Medical Evaluation form prior to start of stay.
*Additional charges may apply.

INCONTINENCE CARE

The benefits of an established continent care management
program are decreased hospital stays, a standard of quality
products, convenience of ordering, and a thorough initial
assessment to ensure the correct size and products are
ordered. This also saves time and gives the peace of mind
that there is an adequate monthly stock.

• Care Package A $335/month (Service)
• Care Package B $445/month (Service & Supplies)

Continence bundles delivered monthly and include:

• Liner pads, pull-ups (similar to Attends) and/or adult briefs
• Gloves (sent in a home-like package to avoid the clinical
  look of a standard box of gloves)
• Lotion or barrier cream
• Aloe Vera adult size wet wipes

• Room Reservation Fee $85-$100 Per Day
• Late Fee $75.00 Payments received after the 10th
• Returned Check $35.00 Per occurrence
• Admin charge $555.00 For stays less than 72 hours
• Emergency Supplies & Services $100-$300 in the event of an emergency.

Transportation:

• Appointments $50 / Hour
• Non - Scheduled local Travel $75.00 Per trip (travel companion may apply if applicable)
• Travel Companion $25.00 /hour – 1 hour minimum

Guest Meals & Room Service:

• Breakfast $10.00
• Lunch $10.00
• Dinner $10.00
• Holiday $15.00
• Resident Room Service $10.00 /Meal

Other:

• Carpet Cleaning $75.00 Per occurrence - spot cleaning
