HOME HEALTH AIDE PACKAGES

HOME HEALTH AIDE VISIT
$9.75 / 15-minute increments
The Home Health Aide provides supportive services identified in your service plan. These services may include medication assistance (when medications are set up by a Summit Hill Living Nurse), safety checks, and assistance with activities of daily living.
MEAL PLANS
- Guest lunch $8 / meal
- Guest dinner $6 / meal
- Room service $3.50 / delivery (in addition to meal cost, waived if resident is ill or recovering)
MEDICATION PACKAGES
- PACKAGE A: Two Home Health Aide visits per day to assist resident with medication. This also includes medication set-up by a licensed nurse. $613 / month
- PACKAGE B: Four Home Health Aide visits per day to assist resident with medication. This also includes medication set-up by a licensed nurse. $1,080 / month
These packages may be appropriate for clients who have few care needs yet need assistance with medication compliance. If a resident requires the staff to spend more time than prescribed in the above packages, the resident will be charged $9.75 / 15-minute increments for the Home Health Aide and $22.25 / 15-minute increments for the Licensed Nurse.
MEDICATION SET-UP
$39 / week • Insulin add $15.50 / week
A nurse will place medications in individualized, labeled containers for each client and will place the containers in the client’s apartment. The service includes coordination with the physician and pharmacy for medication orders, compliance assessment, response to medications, and consult with resident or responsible party as needed.
MEDICATION CHANGE
$17
A nurse will adjust the medications to comply with the revised medication order. The nurse will reset the medications in the individualized, labeled containers.
NURSE VISIT
$44.50 / 30-minute minimum $22.25 / 15-minute increments thereafter
The nurse will provide services ordered by the physician and agreed to in the service plan. Nurse telephone consultation evenings, weekend, or holidays: $15 / call (physician or family contact)

- ASSISTED MEMORY SUPPORT $2,118 Resident cooperates in assisting with routine cares such as dressing, grooming, bathing, and toileting. Needs assistance with medication monitoring. Resident’s behaviors may require occasional staff intervention.
- MODERATE MEMORY SUPPORT $2,675 Resident may be incontinent of bowel and bladder. Resident requires direct assistance of one staff person with dressing, grooming, bathing, medications, and/or management of incontinence. Resident’s behaviors may require frequent staff intervention.
- ADVANCED MEMORY SUPPORT $3,190 Package includes moderate memory support and assistance of a two person transfer, or a sit-to-stand device.
