2.97 out of 5 stars. Based on 442 reviews
Employees of Ensign Services report a great environment, benefits, and friendly staff. However, they also mention a fast pace, location issues, lack of competitive pay, poor management, limited resources for help, low pay for the work they do, rude residents, lack of structure and non-defined responsibilities, lack of transparency, nepotism, and a lack of appreciation for employees. Advice to management includes being more empathetic and listening to employees, putting patients first, not profit, hiring for talent, not just bodies, and creating more lower level office staff to help ease the burden off administration and nursing.Information from Centers for Medicare & Medicaid Services

My father-in-law was there for one month following a fall with a neck fracture. Night staff ignored him and would not answer the call button so he could get assistance to the bathroom. He kept falling because of this. They lost his hearing aid and refuse to return calls or transfer us to someone with authority to tell us when they will replace it. My husband had to threaten legal action to get anyone to call us with his health updates and my husband is his medical power of attorney. Father-in-law was calling everyday begging us to get him out of there.
Every single person at this facility was kind, approachable and compassionate towards my Dad and myself. I have had my parents in other places and I can tell you with full certainty this is the only place I would recommend. The communication between myself and the staff was excellent. The care they provided was above and beyond (bringing in hair clippers to trim my Dad's facial hair and hair). Providing clothes to him when he didn't have clean clothes. Dr. Raju, the staff doctor, was thorough, considerate of all situations and available to speak to me when needed. Communication is so important and Sabino Canyon Rehab does a great job.
This facility did not meet any of our expectations, despite it's 5 star rating. We selected it for dad's rehab after a fall. When he was discharged I was assured that he would be able to live independently, but he couldn't even support his weight with a walker and was worse than when he went in. To make matters worse, they had prescribed a medicine that he is allergic to and missed that on his chart. Fortunately he didn't seem to have any reaction. Unsafe and dishonest. I would give it no stars, but the nurses genuinely seem to care.
I fail to understand how this can be a "Five Star Medicare" facility because of their staffing ratios. I put the light on to get assistance for my husband and waited 40 minutes. Rarely did I see staff in the hallway because they are so busy they cannot get to everyone in a timely manner. The food was awful. I ate lunch with my husband almost every day he was there so I speak from experience. A couple of times I brought in lunch for him so he could have something more palatable. The structure itself is ancient. The bathrooms are so dimly lit, it is unsafe. One time there was a huge puddle on the floor because the ceiling was leaking with rain water. A resident could have slipped and fallen and of course the facility would blame the resident. If medications were discovered in the bed, it was always the resident's fault because they failed to put the med in their mouth. Obviously, the med nurse is too rushed to take the time to be sure the resident took their medication. The only RN you will see is the "Director of Nursing" who graduated from a two year program. The others are LPNs. The Cardiac Rehab they have contracted with Pima Heart consists of the PA dashing in the listening to the resident's heart for 10 seconds. If there are no complaints then off he goes. I don't think anyone should consider placing their loved one in this facility. I am sure there are far better ones out there!!
I can't express my anger in a valid way through this post...so stay with me. My loved one was sent to Sabino Canyon after she had suffered a stroke. During her stay, she suffered another stroke. The hospital sent her back to Sabino Canyon again after discharging her. Her stay at Sabino Canyon was fairly short (about 3 weeks) and then they told us she was ready to be discharged and placed back into INDEPENDENT LIVING. She could not see from the stroke, could barely walk and was having memory lapses. They evaluated her and deemed her fit for independent living. The day after she was discharged, home health nurses found her sititng in her walker, naked except for a sheet and she had soiled herself. Upon being helped to the bathroom, she soiled herself again (on the way). When I entered her apartment two days after she was discharged (I live out of town), I noticed stacks of pill cards laying all over the folding table. She did not know who I was and was generally confused. I called Sabino Canyon for an explanation as to why she had been released in such a state and they told me she had been dressing herself and could walk 100 ft. unaided by a person. They did not see the lack of cognitive skills that everyone else saw and denied doing anything wrong. Did I mention that they sent her home without discharge instructions but 15 different kinds of medication that she had no idea when and how often to take? Well, she ended up back in the hospital, due to internal bleeding as well as overdosing on the medications. When they stabilized her, we put her at Handmaker, a much better facility. Whatever you do, if you love and care for your sick friend or relative, DO NOT ALLOW THEM TO BE SENT TO SABINO CANYON.
Terrible. Do not place your loved one here. My dad got out of his bed this morning and had been lying in his own urine and feces. They discharge at their own discretion and fabricate criteria to attempt to justify it. And we were provided with a 12 - hour notification of his discharge. Really. ......we're supposed to find another place for him with a 12-hour notification? Neglect for the patients and complete disregard for the family. Stay far away! I would not even give it one star if that were a choice.
I'm stunned to read those other reviews, and I'm happy to say that our experience was the complete opposite. My husband needed extended care folloiwng a hospital stay to deal with an infection surrounding his artificial hip. We began at The Forum, which looks SO PRETTY from the outside, and we left after 1 night--1 nurse screaming at my husband's roommate seemed pretty par for the course there. We followed the doctor's recommendation and contacted Sabino Canyon~~I toured the facility and was very impressed with the atmosphere, PT/OT area, employees, and good attitudes of the patients. They helped get my husband out of The Forum and into Sabino Canyon, and the next 3 weeks were filled with kind people and good care. After the next surgery, we'll be returning!
My friend was sent here. It's not a welcoming place. Very small old very crowded rooms. Staff seemed uncaring. Food was not good, my friend requested apples but never got any, so I brought him apples. My friend was very unhappy there. I always left very depressed and worried for my friend. I'm just glad he didn't have to stay there very long. I hate being negative but that's how I saw it.
I have spent the last 3 weeks at Sabino Canyon completing some on the job training and I just wanted to share some of my experiences during this time. First off, they have an established leadership team and management longevity. This is likely directly related to their onboarding process, as they really care about their staff here that allows staff to really grow roots. It's been fun to watch "sabino take care of sabino". The staff here really do care and are very knowledgeable about their patients. The physicians that round are very involved and accessible to the staff. So far I like what I'm seeing and look forward to spending more time with this team. I would highly recommend Sabino for rehab placement and/or as a potential career move.
Do not recommend this place for anyone. My grandmother had been placed here after ehavinf a heart attack. During the night she couldn’t breathe and called for help. They only came after her roommate called for help. After they wouldn’t give her any oxygen and wanted to give her a tylenol. Never send your loved ones here.
If I could give Zero stars I would! My mom is currently there and is so depressed she is crying. Has 25 minute waits after pressing call button, apparently not given a menu because she is repeatedly given chicken when she does not eat any poultry, when questioning a nurse earlier today about what meds she is being given the nurse told her that she was going to put in her medical chart that my mom refused her meds which is not true at all!!!!! She was given oxycontin or oxycodone when there is a note in her medical record that she can not take the opioid and has told the nurses she can only take Tylenol!. The same nurse told her that when someone asks for pain medicine they "automatically" give the patient an opioid because they THINK that is what the patient is asking for! Do Not send a loved one to this HORRIBLE facility!
I've been working at Sabino for two years. I have never met so many caring people who really do treat the patients well. I've worked in many different types of medical establishments, and I can honestly say that the staff at Sabino really do treat patients there with respect and care. The staff treat patients and each other like family. I love this place!
Horrible, rude staff. Not clean. My BFF was there 2 days and signed herself out. She was quarantined the first day but they put some other patient in with her on day 2. The other person kept throwing her drinks on the floor and they didn't clean it up. They put the other person's dirty bed clothes on the floor and left them. They wouldn't come help her to the bathroom. She sent me a picture of the food and it was awful. People in 3rd world countries eat better than that. Terrible place. Stay away.
This facility did not meet any of our expectations, despite it's 5 star rating. We selected it for dad's rehab after a fall. When he was discharged I was assured that he would be able to live independently, but he couldn't even support his weight with a walker and was worse than when he went in. I would give it no stars, but the nurses genuinely seem to care.
If I could give this place a zero I would, that's what it deserves. From the time you walk in the door, the place smells like mildew or mold, they just throw you in a bed and that's it, you might hear from someone the next day! Don't bother with the bell, the light will stay on for an hour with no response. Please don't ever send a love one here, it's a horrible place to be!!!
I'm very sorry, I misjudged you. You had my mother for a month, wouldn't let me visit, and I thought for sure it was because you weren't doing anything and didn't want anyone to know what was going on. She came home today and she's walking better than she has in a very long time. I thought that my walking her every day was enough to keep her strong. I fed and feed her a cup of spinach every day to keep her muscles strong. I thought mylove, would keep her strong. I don't know what you did to help her, because I wasn't allowed to be a part of it. I do appreciate what you've done, because she is all the better for it. Thank you.
When I arrived I was told all the walkers were locked up and there was not one available for me. I am handicapped and can't get around without one. Then I asked for a bedside commode because I couldn't get to the restroom. I was told they were all locked up. The staff didn't show me the call button and disappeared for the entire night, so when I needed to relieve myself I had to do it on the bed. They couldn't get the TV to work until the next day. I told them every day I was there I was extremely uncomfortable and in pain and they ignored me. I asked to sit in a wheelchair for awhile and was told they were all locked up. Most animals get better treatment than I got at Sabino Canyon Rehab. Horrible place.
Administrative Staff and Medical Records are incompetent. Personally had to confront Medical records director, and Administrative director to receive completed medical records for both parents who were in their Covid ward for almost a month. Then after receiving their 1st Covid shots they claimed they gave them a vaccine card, to individuals whom did not even remember what day it was at the time. Had to confront them to get the medical record of the vaccine they received so they could get their second shot, after the Medical records director said they don't keep those records. Dealing with the fact now that my father still has not received his second dose. He is currently in a Hospice situation.
