Furthermore, it boasts a sturdy 4-star quality rating, ranking in the upper half of all facilities when it comes to quality. Despite a slightly lower ranking compared to the state average in this particular aspect, Riverwood Health Care Nursing Home still promises an above-average service.Staff Ratings
Riverwood Health Care, a nursing home, excels in providing its residents with superior care as it is ranked in the top 20% of facilities when it comes to nursing aid staffing hours per resident per day, offering 2.78 hours, markedly above the state's average of 2.51 hours. Moreover, with an impressive record in LPN staffing, placing in the top 10% of facilities, it ensures 1.36 hours per resident per day, outdoing the state average of 1.21 hours. However, it falls behind in RN staffing hours, ranking it in the bottom 30% of facilities, by providing 0.37 hours as opposed to the state's average of 0.57 hours. Despite this, Riverwood ranks among the top 30% of facilities for licensed staffing hours per resident per day, providing 1.73 hours, narrowly trailing the state's average of 1.78 hours. The facility finds itself in the upper echelon of institutions, specifically in the top half, in terms of ensuring their residents remain up to date with the latest vaccinations. Despite the impressive ranking, the resident vaccination rate of 66.7% sits marginally below the state average of 68.7%. However, when it comes to staff vaccinations, Riverwood Health Care excels considerably, boasting a rate of 71.4%, dramatically surpassing the state average staff vaccination rate of 45.8%. Riverwood Health Care is a standout nursing home with a four-star quality measure rating, placing it in the top echelon of care facilities. Notably, it ranks in the top 20% of nursing homes with the least percent of residents experiencing one or more falls with major injury, boasting a rate of only 0.88%, which is significantly lower than the state average of 1.78%. Riverwood's resident-focused care is also reflected in its low weight loss figures, with only 12.43% of its residents struggling with excessive weight loss, although this rate is, regrettably, considerably higher than the state average of 5.14%. Furthermore, the facility's personalized care support is illustrated by the fact that only 8.21% of its residents have an increased need for assistance with daily activities, which is slightly below the state's 8.52% standard. And in another favorable contrast, only 8.12% of Riverwood's residents require antianxiety or hypnotic medications, a figure considerably lower than the state average of 13.75%. This data emphatically reaffirms Riverwood Health Care's position as a leading nursing home that puts its residents' health and safety first.Emergency and Hospitalization Quality Measures

When examining hospitalizations per 1,000 days, Riverwood Health falls in the lower 20% of facilities, with a rate of 2.62, surpassing the state average of 1.89. In terms of outpatient emergency department visits per 1,000 days, this institution is also situated in the bottom 20% bracket, as its rate of 1.72 is notably higher than the state rate of 0.92. On another note, considering the percentage of short-stay residents who had outpatient emergency department visits, Riverwood again falls below average, ranking in the lowest 40% of facilities with a value of 13.77%, higher than the respective state average of 11.22%. However, Riverwood Health has shown to perform well in terms of rehospitalization after nursing home admission for short-stay residents. I just spent 3 weeks at Riverwood Healthcare Center. in Stockton recovering from a TIA (mild stroke). I wasn't looking forward to it but Riverwood changed my mind. The entire staff is fantastic, they are all loving, caring & professional. They treat you with respect & they treat you like you are part of their family. If I ever have to go back in for physical rehabilitation, I will go nowhere but to Riverwood. I thank, each & everyone at Riverwood for making stay easier on me & for their fantastic care they gave me. I have to make a special shout out to the physical & occupational employees at Riverwood. You guys are the BEST! You really helped me get back my confidence & my strength. I promise you that I will continue to do my exercises, so I can be safe in my home. God Bless You!!!! Going to miss you all. Laura Cranford
I have had the pleasure of visiting Riverwood on two occasions. I was greeted by friendly staff both times. The place looks and smells good. This place really prides itself on providing world class patient care and customer service! I would recommend them to anybody that is looking for a nice, clean skilled nursing facility in the Stockton area.
I do not recommend the this facility for you are your family memeber I was just released from here spent a month and other then the excellent cleaning staff and the therapist Jim and about 5 cna and about 2 nurses this place is terrible the staff will steal your belongings that you came in with overnight staff will have u waiting all day when u press your buzzer the food shouldn't be severed epically breakfast they are very unorganized the staff that don't care don't care at all always make thing your fault when it's theirs all they want to to do is sit in bed all day long always have attitudes about everything I ask for a complaint for 3 times and never got one the showers is in the janitor closet with all the dirty stuff look nice on the outside but very bad Energy in the inside if you do have family that is there make sure you talk to them and always call riverwood to check up on them so they know they have family if you really wanna know how it is talk to your family member and over clients there
Relationships and the way of doing business with a Healthcare Center during these difficult Pandemic times is trying and difficult for both the Hospital and for the patient. What I want to address is the way Riverwood is dealing with patients during this difficult time. I am here at this time due to left sided weakness and rehabilitation to me so has a double amputee. I can get the help I need from the entire staff and specialist that are working with me for the ability to go home stronger to be independent enough to be able to live with Gloria at home with her providing me with minimal assistance. The people here at Riverwood go way out of there way to make us the patient comfortable and allow us to work with them to rehab us. Great staff. Great CNA's and RN's. The therapist both PT and OT are excellent. If you need a rehabilitation hospital. Then this is the place to be. The administration and his Staff are excellent. I really enjoy this healthcare facility. If you have a bad experience here. Then you are not working hard enough with them to get yourself better. I hope you never need these special services but at least you have a great place to choose. Robert T Peterson Room 15 bed A. Excellent service and great food and care.
I have recommended many people to come to Riverwood Healthcare Center for their short term rehab. They are a great facility who provide excellent care as evidenced by their 5 out of 5 star CMS rating. It is a beautiful and clean building that has experienced and competent staff. They have a fantastic therapy staff who work miracles every day! And the building is COVID free!
In looking for a Rehab center to help me get back on my feet...I decided to go to Riverwood...what a great decision I have made...the people...nurses...doctors..physical therapist and also the administrator are fantastic at what they do...they treat me not like a patient...but as family...if you're looking for a family oriented rehab and convalescent home...Riverwood is by far the best place to be...activities and therapy are amazing..the staff here will get you to and from your doctors appointment...with no hassel...taking a lot of the pressure off of your family...i highly recommend Riverwood for your love ones or even your self!!!!
BEWARE CURRENT AND FUTURE PATIENTS AND FAMILY OF RIVERWOOD!!!This is a fair warning before even considering to take your loved ones here. I had a loved one here not too long ago and had the worst experience. Every time I visited my loved one was in bed, soaked and wet, and hungry. When approaching the nurses and cna's, they seemed just as clueless as me almost like a deer in the headlights. When asking the staff when the doctor would be here, we were told the doctor has already came and gone. We've asked numerous times to speak to the doctor but there was always a lack of communication. I tried to speak with social service and was greeted by a young girl who introduced herself as the social worker, Anna-bel. She was clueless, very rude, and as it turns out she's not a degreed social worker and was previously the on-site assistant. Please think twice or even a third time before having your loved one go to Riverwood Healthcare.
FAMILIES AND PATIENTS BEWARE!! If I could make this a zero star rating, I would. YOU MUST BE AN ADVOCATE FOR YOUR FAMILY MEMBERS AT ANY SKILLED NURSING FACILITY, But, ESPECIALLY Riverwood Healthcare Center! My father-in-law was a patient here, my wife is his Power of Attorney (POA).Riverwood was very difficult to communicate with and had little respect for the fact my wife was responsible for making health care decisions for my father-in-law who suffers from dementia. There was a huge lack of communication with/and between nursing staff members, the doctor who we never met, never spoke with us as family. We never had the opportunity to speak with the doctor. Although, in the records from Riverwood it states that the doctor had talked to us. We found out that at the time of our experience with Riverwood, the person who was representing herself as a "Social Worker" for the facility is not a degreed social worker and should have presented herself as a social service designee. We were never made aware of a supervisor with a MSW that we could go to with questions or concerns. There are many issues and concerns we had while dealing with this person. If you or your loved one has experienced problems at Riverwood, let me encourage you to contact the SJC Ombudsman at 209-468-3788. And make a complaint with the Dept. of Public Health 800-554-0354.Also, today in the mail we received a letter that is asking for a 5 star rating on both yelp and google in return we would be entered into a drawing for a $100.00 gift card in appreciation for our time completing the reviews. I personally find this totally inappropriate and unethical. When I look at their reviews knowing this I cannot find them credible. (If I could figure out how to post the letter I would).
My first experience in a healthcare facility was here at Riverwood. I was completely surprised with the service. The nursing staff and nursing assistants were awesome. I had just had open heart surgery and not doing well. My sleeping arrangements were totally uncomfortable making it difficult to sleep. I wanted to be released to go home so I could be comfortable, instead I was moved to a different room with a bigger bed and even a recliner was purchased to help me. The staff was always there to make sure I was ok and taking all my meds. I loved my staff. They made me feel very special. I would recommend this facility to family and friends. Thanks Reginald Goring for making the difference.
The moment you step into this facility you'll experience a unique atmosphere of caring, dedication and professionalism rarely found in most Skilled Nursing Facilities. It's the people who work here that exemplifies that quality of service and exceptional healthcare you expect for your love ones in their critical process of recovery. From the friendliest receptionists who greets you with their welcoming smiles, the competent nurses, the hardworking CNA's, the experienced and skillful therapists, and all the dedicated staff that's always ready to serve above and beyond their call of duty. Not to mention their keen understanding of how important the physical environment fosters the effectiveness of their medical treatments and therapies. This place is clean, well equipped, proficiently managed, suitably employed to provide the best care for patients to recover and return to their optimal health.
Riverwood is one of the top facilities I've come across when considering Rehabilitation services. The Therapy staff is very attentive to patient's needs and returning them home in their highest achievable level of function. As a team, Therapy, along with the Administrative, Nursing, and House Keeping staff, give patients the best possible stay as far as communication, cleanliness, and comfort, that they can recieve at a facility. I would trust a beloved family member as a patient in the care of Riverwood Healthcare Center.
My dear mother was here at Riverwood. First of all there are no phones in the patient's room, they have 2 cordless phones that they pass around so patient's can talk to their loved one's, that was horrible, there were times we couldn't speak to our mother for day's because the phones were in use, and couldn't visit because of covid. My sister and I had to stay on top of the employees and had to have several meeting do to the way my mother was being cared for. It was a total nightmare 😔 I could go on and on about this place the way they treated my mother. If I could give no star's I would. They made to many mistakes!!!! Shame on them!!!
Don't trust the employee-generated 5-star rating! Take the time to go into the community and talk to some of its residents if you want an honest and unfiltered assessment. Be alert, pay attention, and ask questions! Never base your decision on a website. Anybody with basic computer skills can throw together an attractive webpage. The website should never be more inviting than the actual community or the staff. If it is, move along. You want the best care possible, and it's not here.
The food is fantastic the people are happy to get whatever you need even in the middle of the night or day ive had a lot of pain and been a little grouchy at times please forgive me this place i would bring my mom to. great staff i give 5 stars wonderful outstanding clean super clean masks and doing there part to help the comity and all of use thank you for taking care of me when I need it Jamie Thornhill
It’s an ok place but if the patient does not an advocate nothing happens, you have to be on top of everything. There is no communication between shifts, so please if you have an elderly family member in need of a nursing home do not leave them alone, always make sure things are compline completely and in a timely manner.
I had a very good experience at Riverwood. After being there for a few days I wanted to be released because I was not comfortable. Bed was not working for me or the environment. I thought I could recover better at home. The Administrator came to see me to see what he could do. Within 25 hrs I was moved to another room where the accommodations met my needs. A recliner was moved into the room to help with my recovery. The stay was so much more than I expected and the medical staff helped with the recovery. They were awesome.
I’m a CNA and first job was a TERRIBLE experience. We were always understaffed and our management team was rude. After moving to Riverwood, I love working with the staff here. I feel we are all a family. The CNA’s, the nurses, the kitchen staff, the housekeeping staff are so friendly! Our staffing coordinator is amazing at working on the schedule and is always working to help the CNA’s. I also love how our management is approachable and friendly.
