We had some issues with staff and safety issues, Some of the staff was very good-deserve a five stars. But the the big picture is this it all falls on poor Management. Most of the staff is there for a pay check and would lie to your face about stuff. The doors are not locked at eight o’clock night . We showed at nine forty pm UNLOCKED. Several times, They have ANOTHER DOOR off to the outside sitting area that does not lock (broken) , MOLD on the area of the ceilings in hall next to the PT room , This is not a very clean place. The rehab they do short rehab session five days weekends off it did help gain some of there strength back. The average session was about thirty minutes but not a continues 30 minutes. We know there billing insurance for more Mary the administrator never responds back to any type of message that you leave. While visiting we wanted to compliment the good employees and needed to address that we found the oxygen not connected to the tank several times or the oxygen machine not turned on. There was a fall that was never documented , one of the CNA told them they needed to start helping there self and should get up on there own. ( not yet released to do that yet ) Morning medication given at lunchtime. The list goes on. I wanted to address this with corporate office unable to get that information. Wonder what there hiding.
