Second Person Fee: $1,000 (Assisted Living Care Not Included)

*Specific services needed by the resident are determined by an assessment performed by the community. Upon the determination of what services are needed to support the resident in activities of daily living, such services will become part of the residents care (service) plan. Included in the base rent as needed in Memory Care are the following services: Cueing and Re-Directing, Dining, Oversight, Routine Behavior Management, routine Oral Medication Administration and Bathing 2 times per week.
Not included in the base rent in Memory Support and based on individual resident need are the following services; 2-person assist, any ADL or service requiring 2-person assist, complex medication management-any treatments, injections, texture modification, Diabetic or Coumadin Management or resistive to oral medications, feeding-requires physical assistance with feeding during meals.
Additional care (service) charges for the above will apply, based on assessment.
