Do you guys currently have a waitlist?
Yes, we have a waiting list that is approximately 3 years out.
Could you share pricing and any other general information?
We are Currently $480 a day, each year the price increases.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
DPH Mandated training and CMS training, Dementia Training as well as Trinity Health Mandated trainings.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
All doors are locked and individuals coming in have to ring a door bell and staff must let them in and we also have to buzz them out.
Do you offer any resources or support services to aid residents' transition into the community?
We have a Social worker on sight and Behavioral health that comes in every 2 weeks.
Do you offer transportation services for residents?
No as we are a skilled nursing facility.
Could you tell me a little about your dining experience?
All three meals are served Family style, all residents on the unit come together at the dining room table.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
We have a grievance policy in place that goes through the social work and the administrator.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We have care plan meetings every 3 months, and you can always contact the nurse on the unit for an update, or if your loved one has a change in status you will be notified by the nurse.
Do you all offer short-term or respite stays? How much does this cost?
We offer rehab stays and LTC. Our Daily rate is $480 a day.
Are you all a pet-friendly community?
Dogs are allowed to visit a resident, but must have vaccination papers. They are not allowed to live with a resident.
Are residents able to get Covid vaccination and flu vaccinations in the community?
We offer Vaccines at the facility they do not need to go out.
Are residents able to age in place?
Yes, as we are a LTC care skilled nursing facility.
