BASIC RATES
Assisted Living Rates
- Studio (298 square feet) $5,160
- Suite (520 square feet) $5,540
- Additional $2,635 for couples
Assisted Living additional services
Basic rate + additional $750 a month for each additional service
Upon admission, the Administrator and Case Manager will consult with your primary physician to determine whether, given your care needs you meet the following criteria;
- Receiving dialysis treatments, transportation to and from Dialysis treatment three (3) times a week, diet and weight.
- Insulin Dependent, requiring the provision of insulin injections and diabetic monitoring, including nursing assistance, as required, to the resident in the use of a glucometer, logging results, follow-up tests if outside of physician parameters and physician follow-up both as required and at least weekly, filling syringes or assisting with the dose for those residents who use pens for injection, providing injections as needed.
- Daily blood pressure An-LPN providing the provision of daily blood pressure checks, logging, notifying physicians, follow-up.
Special Needs:
Private Rooms $7,195
Enhanced Tier I on Assisted Living Side
Current ALR rate +$580 Assistance with any of the following: Administering eye drops, ear drops and/or application of medicated creams and ointments.
Enhanced Tier II on Assisted Living Side
Current ALR rate + $2,195 Assistance with any of the following: Administering of injections, the taking of vital signs such as temperature and blood pressure, skilled observations which need to be reported to a physician, PRN medication administration, wound care, finger stick testing of blood sugar levels, oxygen care/nasal cannula, care related to those using an oxygen cylinder and concentrator, nebulizer treatment and assistance with toileting.
Enhanced Tier III
$9,175 single occupancy: includes any services from Tier I or Tier II, indwelling foley catheter care, condom catheter care, colostomy care, suprapubic catheter care, bladder/catheter irrigation, nephrostomy tune care and one (1) person (certified staff, HHA) assistance with or without a gait belt for transfer and/or ambulation.
2nd person Enhanced add an additional $5,375
2nd person Assisted add an additional $2,750
Enhanced Tier IV on Enhanced Side
$11,800 single occupancy: includes any services from Tier I, II and/or III and two (2) persons (certified staff, HHA) assistance with or without a gait belt for transfer and/or ambulation.
2nd person Enhanced add an additional $5,375
2nd person Assisted add an additional $2,750
Enhanced Tier V on Enhanced Side
$12,930 single occupancy: includes any services from Tier I, II, III and IV and the use of a mechanical lift and/or supervision/assistance with feeding by an HHA, LPN or RN
