I was there maybe tops 4-hours. The place was dirty. The CNA’s were slow and rude. Lorraine had an attitude and would just stare at me without responding, I asked if she heard me and she did she just didn’t want to be bothered. The other CNA was forceful and pushy. She wouldn’t listen to what I was trying to explain to her of the instructions I was given by the PT on how I was to walk to the bathroom and because of her ignorance I fell. Lorraine’s nose was dripping on me occasionally, disgusting. When I fell and the RN got to me I had enough I told her to get me the hell out of there! She sent me to the hospital to get X-rays thank god I didn’t break anything but my feet were badly injured which has set me back in my recovery a week. This facility is a nightmare. I wouldn’t trust them with a dead body. My experience there was horrible.

It is so dirty here. The furniture is so gross and the upholstered chairs are filthy, with stain upon stain on them. My diabetic father is staying here and we have asked them to provide milk or Glucerna at each meal and they've ignored it and continue to bring him juice. They don't seem to care about his dietary needs. Not impressed or happy. I don't understand why this facility has 4 stars.

Don't bring your loved ones here if you care about their quality of life. I am so livid with the level of neglect by the staff here and how they treated my beloved grandmother. The one instance that stands out to me most clearly, one night I was visiting late (around 9pm) and as I was leaving, I asked the CNA on duty if she could please go help my grandmother change into her pajamas and get into bed. She said she would be right in. When I returned early the following morning, I discovered my grandmother was still wearing the same clothing she had on the night before - even her shoes were still on! And she was laying on top of the covers with only a throw over her to keep her warm. Furthermore, she was on a strict renal diet due to kidney failure, but Life Care Center does not accommodate that apparently. Everyone just gets whatever they're serving that day regardless of dietary needs, and they fed her a lot of foods she was not supposed to be eating. Another time, I overheard one of the staff members refer to my grandmother as "What's-Her-Face in room 203." Let's just say I've spent a lot of time in my life visiting loved ones in assisted living facilities, and I have never been more dissatisfied with the quality of care as I have been with this place. I wish there was more I could do about it.

At LIFE CARE CENTER in salt lake city utah (millcreek) they care about 3 things !!! 1.MONEY 2.MONEY 3.MONEY having my Mother in there for over a year it's was an absolute stressful and depressing time trying to deal with the BILLING OFFICE people having been given wrong information from the first day and doing the mountain of paperwork and constantly being talked down to and constant arguments with them they profess to being expert KNOW IT ALLS on the social security and medicaid system at times they were wrong and they kept sending invoices to my parents address all the while knowing that there was nobody at that address and trying to do a medicaid annual review and having to dig up mountains of paperwork it was never enough causing medicaid to sanction my mother even though they agreed to cover my mother through the COVID 19 CRISIS that wasn't good enough for life care so the decision was made to move my mother to another care facility and still receiving my mother's social security benefits after she was moved to another care facility the refused to forward the funds to the new care facility causing another huge argument again (MONEY) Although this is only my opinion it is very important to do your homework before making the difficult decision to place a loved on in a care Center anyway I'm happy that I never have to deal with Life Care center again

We have been extremely pleased with the care our Mom is receiving. The employees have been friendly, professional and dedicated. When my mom came in she couldn't even sit up without help. After two weeks she's able to walk (with assistance ) to the bathroom and down the hall. Our family would like to express our profound appreciation for all the hard work and most of all for the kindness and support they've given us and our mom.

Very caring CNAs and nurses. However I worked there for over a year and made less than people with much less experience. However I believe they are working on that. I was treated really well by management and the NOC shift has excellent staff. They are making changes trying to make the place better. The only reason I didn’t give it 5 stars is no facility is perfect. But it is a good place.

His care was given by kind and caring staff.So when I needed a place for my mother the choice was easy. I have been impressed by their ability to listen to my mothers wishes, and meet her needs. I'm currently interning at this facility and have loved it! The staff are so friendly and truly care about each resident. The facility has an ice cream parlor, a big bird cage with a few little birds for the residents to watch, and a cute little dog named Huck that hangs around at scheduled activities. From what I've seen, the residents enjoy their time here and the staff have become family. I would highly recommend this facility to any family member in need of a SNF.

I liked all the nurses and thought they were all very helpful, and made sure all my needs were taken care of. The physical therapy I went through was great. They knew exactly what I needed to do and helped me to have a little fun while preparing me to go home. The breakfast was good.

I don’t think my Dad was sure he wanted to leave when he was strong enough to do so because he was taken such good care of! I never heard him complain about anything the entire time he was there. He was happy with the faculty, Nurses, the therapy provided, and all the others who were a part of his recovery team. He said the food was great as well. The facility was always clean when I visited & I was happy to see that it was a secure & safe environment. I would recommend Life Care Center to recover in!Nora Salzman

The staff and team at Life Care of Salt Lake are first class! They care about their residents and go above and beyond to meet their needs. Craig, Megan, Nick, Kyle, and Brent have always made me feel welcome in their building and have always tried to accommodate any needs I have. 5 stars for this amazing facility!!!
