Unfortunately, we do not provide telephones in our resident rooms, however phone services are available through Windstream and are billed privately to the resident or family member. You may call the telephone company @ 1-800-347-1991 for the cost of hook-up and monthly rates.
We do provide Cable Television, free of charge, for all residents.

Overview of Professional Staff and Training
Lake Park employees are licensed Registered Nurses, licensed LPNs and certified nursing assistants that provide direct care to our residents. There is a Medical Director, an administrator, and Director of nurses along with other supervisory nurses among our staff.
Safety and Well-being Measures
The facility has monitored entry and exits via the front lobby. We do not tolerate abuse, neglect, nor misappropriation of resident funding. Patients/family are responsible for communicating with the patient’s assigned nurse.
Transition Support Services
There is an expected transition period from 5-14 days when a patient admits to adjust that might require medicines, therapies, or interventions.
Transportation Services
Scheduled transport to patients’ appointments are available but the patient/family are responsible for the information communicated during the appointments due to the privacy policies.
Dining Experience
We offer family style seating with dinner trays. Residents have the ability to dine in their rooms but are encouraged to eat in the dining room areas. Individualized diet orders are required for each resident.
Resident Complaints and Issues
We have a grievance policy that we follow with every concern; this is available in our resident handbook.
Cost of Living

Communication with Residents' Families
This standard is established with each family/resident on admission so that the effort of managing care is not lost.
Short-term or Respite Stays
Respite stays are available and they would be same as the daily room rate for as many days as needed.
Pet-Friendly Community
Yes. Pets are welcomed to visit but not overnight. Please provide the facility Activity director a current copy of the pets vaccinations record.
Covid and Flu Vaccinations
These vaccinations are offered.
Aging in Place
Patients can age in place as long as there are NO acute episodes requiring an acute level of care. (Hospice and Palliative management are offered to effectively manage MOST acute episodes.)
