Assisted Living Pricing
Our assisted living apartments are offered on a rental basis, with inclusive monthly fees that generally cover 80% to 90% of one’s cost of living expenses. Our assisted living lifestyle includes:
- Choice of studio, one- and two-bedroom apartments.
- Health monitoring by a licensed nurse 7 days a week
- Trained care associates available 24 hours a day
- Three (3) meals per day with emphasis on nutrition, including MIND Diet selections that promote brain-health
- An always available menu
- All utilities (excluding telephone)
- Weekly housekeeping and linen service
- Scheduled transportation to medical appointments, shopping, and off-site recreational outings
- Enriching activities and social events
- Wellness promotion and exercise programs
- Emergency response system

An individual service plan is developed for each resident based on a care assessment from our licensed nurse. Personal care might include: dressing, bathing, transfers, escorts, reminders, medication management, incontinence management and treatments.
There is a one-time community fee of $3,000 per apartment. For apartments occupied by more than one person, there is an additional charge of $1,000/$1,000 per month to cover additional meals, apartment and building maintenance, insurance costs, activities, and scheduled transportation expenses.

Independent Living Pricing
Keystone Place at Forevergreen offers one and two bedrooms/two-bathroom independent living apartment homes. Apartment features include:
- Fully-equipped kitchen with refrigerator, range/oven, microwave, and dishwasher

- Spacious bedroom(s)
- Bathroom(s) with non-slip shower(s) and safety grab bars

Our memory care program features:
- Innovative programming designed to engage residents in purposeful activities
- A secure setting with a private outdoor courtyard
- Neighborhood designed for familiarity
- Private apartment-style living
- Access to individualized personal care services (i.e., bathing, grooming, dressing, eating, toileting, ambulation, etc.) and medication management
- Health and wellness monitoring by a licensed nurse

Nursing and Care Staff
We have 4 nurses, 1 part time and 3 full time in our building. We also have certified medication passers on each shift. Nights and weekends we always have a nurse on call. Everyone in our building has ongoing training including dementia training. (This includes our kitchen staff, and housekeeping staff).
Safety and Resident Checks
We follow state rules and regulations for safety and go up as high as the state allows for cares. We also do a “behind the scenes” daily check of our Independent residents. We have to see them once a day, if we don’t, we call or knock on their door to make sure that they are ok. We see our assisted residents more often, (all meals) but we go around with a hydration cart 2 times a day to check in on them as well.
Resident Relations and Community Integration
We have a full time resident relations person that helps ease the new people into the community, we find out what activities they will be interested in and the first month they can push the pendant and someone will take them (we don’t want anyone feeling like they cant find something and not go.) After the first month, they can add this service onto their care plan. We also make introductions to our more settled residents so they can make connections. I always invite people to start coming to activities before they move in, like coffee chat to start making those connections.
Transportation and Outings
We do have transportation to doctor appointments Monday – Thursday and Fridays we have outings for the residents. This Friday they are going to a Mexican place for lunch and then to Walmart. They can also sign up for Friday “open slots” so they can go wherever they want in the area. (Dollar Tree, JC Penney are popular).
Dining Experience
We do not have to adhere to any special dietary needs so we have a wonderful chef, residents choose from the menu when they come down to the dining room, they are waited on by servers (it’s like a restaurant) if they don’t want the special of the day, they can choose from an alternate menu (burgers, chef salad, fish and chips, club sandwich, etc).
Feedback and Communication
We don’t have a lot of complaints, but we do have a monthly chef chat where residents can give the chef feedback on things they liked and didn’t like and what they would like to see on the next months menu. We also have a director chat once a month where the residents can ask questions, (when will windows be washed, what is going on with hiring and the occupancy of the building.) One thing I like is that all of the directors offices are in one general area, so it is easy to find someone to share a concern with. We are a family owned business, not corporate and we are very resident driven so the answer is pretty much always “Do what is best for the residents”.
Family Communication and Social Media
We get emails from the residents families when they move in, so if there are major changes (like our DON on maternity leave) we send out emails. We also encourage them to connect with our Facebook page, we post a lot and that is a good way for them to see the activities that are going on and the menu for the week.
Respite and Accommodation Options
We do offer respite, minimum of a month stay and it is $200 a day. We are month to month, so nothing is ever permanent if you don’t want it to be, however since we are very resident driven, we keep a very high occupancy.
Pets and Amenities
We love pets here! We have lots of dogs and cats in the building and even have a dog park!
We do offer Covid and vaccination clinics, a podiatrist also comes in once every 3 months.
Living Options and Therapy Services
We have Independent living, assisted living and a 20 unit memory care, we do not have Long term care or Skilled Care. We do have a therapy company that is housed in our building, so residents can get PT,OT and Speech therapy if needed.
Availability and Waitlist
I think that is all! Let me know if you have any other questions, We do currently have a waitlist, we are about 2 months out for an assisted living apartment and about 4-6 months out for an independent apartment. 2 bedrooms can be a bit longer of a wait.
