Overall Ratings
Wesley Manor Nursing Home boasts a reputation that places it among the top half of facilities in light of its overall rating, which incidentally stands slightly higher than the state average. A testament to its dedication to maintaining high health standards, Wesley Manor has earned a commendable 4-star health inspection rating, making it among the top 30% of facilities in this category and resulting in a health inspection rating which is higher than the state average. However, the facility does fall short in its 1-star quality rating, which places it in the bottom 10% of such facilities. This quality rating, tagged as the lowest possible, lingers below the state average.Staff Ratings
Wesley Manor nursing home is acknowledged for its exceptional staffing record, especially in nursing aid staffing where it is ranked in the top 30% of facilities, boasting an impressive 2.42 hours per resident per day, exceeding the state's average of 2.28. However, the facility struggles with its LPN staffing, being in the bottom 20% with 0.51 hours per resident per day, less than the state's 0.88. Nevertheless, the nursing home excels in its RN staffing, ranking in the top 20% with 0.89 hours, surpassing the state's average of 0.79. Meanwhile, the nursing facility finds itself in the lower half regarding licensed staffing hours per resident per day, with 1.39 hours, falling short of the state average of 1.67. In terms of physical therapist staffing, Wesley Manor holds its head high in the top 40% with 0.07 hours per resident per day which is higher than the state's average of 0.06.Covid Data
Wesley Manor, a notable nursing home, ranks within the top 40% of facilities for maintaining an up-to-date vaccination rate among its residents. With 73.4% of its inhabitants diligently keeping up with their immunizations, the home fares significantly better than the state average of a mere 60.8%. This accomplishment demonstrates the institution's commitment to prioritizing the health and wellbeing of its residents. However, the picture is less positive when it comes to the staff's vaccination rate as it is placed among the bottom 20% of facilities. Falling significantly below the state average, Wesley Manor has a vaccination rate of only 3.5% among its staff, compared to the state average of 15.6%. This disparity indicates that while residents' health is well-maintained, there is a dire need to improve the vaccination rate among the facility's personnel.Quality Measures
Wesley Manor, a nursing home, has unfortunately received a 1-star quality measure rating, putting it at the lowest possible ranking. Among the concerning issues, 25.42% of its residents have seen a worsening in their ability to move independently, a percentage significantly higher than the state's average of 16.18%, placing the facility within the bottom 20% of all establishments. Its use of antianxiety or hypnotic medication is also alarming, with 34.94% of residents having received such treatment, exceeding the state's average of 29.47%. This places Wesley Manor in the unsettling position of being in the bottom 10% of facilities in this respect. Additionally, it falls into the bottom 20% concerning residents having one or more falls with a major injury, with a rate of 6.32% surpassing the state's average at 4.15%. However, when observing depressive symptoms, Wesley Manor's percentage of residents suffering is 3.89%, which is lower than the state's rate of 9.07%, placing it in the bottom half of facilities regarding this measure.Emergency and Hospitalization Quality Measures
Wesley Manor, a nursing home, finds itself within the lower 40% of facilities concerning hospitalizations per 1000 days, with a rate of 1.91. This figure, though lower, closely mirrors the state's rate of 1.92 hospitalizations per 1000 days. On the other hand, Wesley Manor boasts a position in the top 40% for outpatient emergency department visits per 1000 days, which averages 0.77, significantly outperforming the state's average of 1.56. However, when considering the percentage of short-stay residents who visited the outpatient emergency department, Wesley Manor falls within the bottom 30% of facilities with a rate of 16.29%, which is slightly higher than the state's percentage of 14.24%. In addition, Wesley Manor is in the bottom quadrant, more specifically in the bottom 20% for the percentage of short-stay residents who were rehospitalized after a nursing home admission, standing at 30.39%, which is worryingly higher than the state's percentage of 22.63%.Healthcare Nearby

EXTREMELY UNPROFESSIONAL.. MY MOTHER & I HAVE BEEN TRYING TO GET MY DAD OUT OF THE HOSPITAL FOR 2 DAYS ... AFTER NO RETURNED PHONE CALLS YESTERDAY OR TODAY, I DECIDED TO JUST GO TO THE FACILITY AND SPEAK FACE TO FACE WITH SOMEONE TO FIND OUT THE PROBLEM . AFTER WAITING NEARLY 30 MINUTES, NIKI THE DIRECTOR OF NURSING APPEARS AND SAYS "SHE AINT GOT NO PAPERWORK. AND AINT HAD TIME TO RETURN NO PHONE CALLS" because he was technically dismissed yesterday, he HAS to now leave today. SO NOW INSTEAD OF BEING IN OUR NEIGHBORHOOD LITERALLY AROUND THE CORNER, HE'S GOING TO BE FARTHER AWAY FROM HOME FOR HIS REHAB STAY. THEY REALLY NEED TO GET IT TOGETHER. CAN I GIVE A NEGATIVE RATING? THIS HAS BEEN ALL BEEN SUCH A JOKE.
This facility is amazing! The staff is caring and respectful to their residents. The activities offered for the residents show that the personnel responsible care about the people and their welfare. I am not a resident, but I would trust that my love ones would be cared for at Wesley Manor Retirement Community.
Shout out to the staff in memory care. They exceeded our expectations! Special thanks to George and Jerri. Everyone was great. Clinton, his assistant, sorry her name escapes me at the moment, even those that did the cleaning. The Lord has a special blessing for you I am sure. Mom has been moved to health care recently and you are a hard act to follow.
BIG thank you to the staff at memory care. My mother was treated with kindness and respect. I am grateful. The whole staff was always happy to help and go the extra mile. The unit was clean and the patients were also. There were plenty of activities and things to do. Trusting someone else to take care of family is not an easy choice but these memory care staff stepped to the plate and I am very thankful!
I can not say enough great things about the staff in the memory care facility. They took such great care of my Memie. I could tell that they genuinely loved her. We were so blessed that Memie was able to stay in her home. I will never forget WMMC for that. Every one of the staff was amazing with her, they truly did treat her and me like family. I miss all of the wonderful residents and hope to have enough strength to come and visit some day soon. Missing the employees goes without saying. Memory Care does indeed create a family atmosphere - each and everyone of them will always be in my heart!
My mother has been a happy resident at Wesley Manor for a little over a year. She recently experienced a major health issue and the staff at Wesley Manor have been exceptional in helping us and her. They are incredibly compassionate and caring as well as invested in the best possible outcome for my mother. We are so grateful that Wesley Manor is her home.Paula M.

The residents here are happy and it shows. There are many activities to be involved in regardless of ability. The dining room is ran superbly and the food is great. The tenure of the employees at Wesley Manor is unparalleled with staff members having 15, 20, 25, 30 and yes even 40 years of service at Wesley Manor. This shows personal commitment and dedication. When my time comes I hope to be a resident at Wesley Manor.
wesley manor retirement home & rehab is extremely unrpofessional.. my mother & i have been trying to get my dad out of the hospital for 2 days ... after no returned phone calls yesterday or today, i decided to just go to the facility and speak face to face with someone to find out the problem . after waiting nearly 30 minutes, niki the director of nursing appears and says "she aint got no paperwork. and aint had time to return no phone calls" because he was technically dismissed yesterday, he has to now leave today. so now instead of being in our neighborhood literally around the corner, he's going to be farther away from home for his rehab stay. they really need to get it together. can i give a negative rating? this has been all been such a joke.
