Raybrook Manor - Levels of Care
Raybrook Manor has several levels of care available. In our Home For the Aged we provide:
Level One Living
Designed for seniors who are independent with their self-care and room care. This area provides three meals per day, all utilities (except for a telephone), a full activity calendar, access to all amenities, cable TV, and access to our Resident Health Services. Level One Living is a nice bridge between being completely independent and assisted living with just the right amount of support and assistance to keep seniors healthy and active.
Level One Living rates range between $2,790 - $6,030* per person, per month.
Levels 2-5 Assisted Living
Designed for seniors who need some assistance with self-care and room care. This area provides the same services as Level One Living as well as assistance with personal care, medication management, room care, and laundry. Personal checks, emergency call system, assistance with bathing, linen service, and bed making are included at no additional charge.
Assisted Living rates begin at $5,460* per person, per month.
Memory Care
Designed for seniors who are experiencing memory loss. This area provides all the services offered in regular Assisted Living as well as a secure environment where residents may not leave on their own as well as an increased amount of structured activity programming.

Overview of Staff and Training
Our assisted living employs LPNs as nurse managers, and we have Resident Care Assistants (RCAs) as the daily caregivers. RCAs who wish to become medication-passers undergo special training with our education team. RCAs receive training from our corporate education team and then shadow other RCAs on the job. New LPNs train with the team in place.
Safety and Well-being Measures
Our entry door is secured from 8pm – 9am, with access only by use of a keycard or by calling staff via intercom. Resident rooms all have locks on them.
Transition Support Services
Our Life Enrichment (activities) team is notified prior to a person moving in, so they can introduce themselves and learn more about what sorts of activities a new resident might be interested in. We have a chaplain/social worker who can meet with residents.
Transportation Services
No, we do not offer transportation services for residents. There are a few organized bus outings each month with Life Enrichment, but we do not offer transportation to appointments.
Dining Experience
We offer communal dining and provide 3 meals/daily. If a person wishes to eat privately in their room, we can accommodate that as well. We also have a restaurant that serves lunch M – F, and is an option if a resident wants something different. Those charges would be added to the resident’s bill.
Resident Complaints and Issues
We encourage residents to bring their concern to their unit manager or the Executive Director, who in turn will notify the appropriate party to rectify the issue. There is also a formal complaint process which can be made to the Michigan Department of Consumer and Industry Services if a person feels their complaint needs to go further. Residents are provided with this contact information when they move in.
Cost of Living
The cost of living in our community has been provided in the previous attachment.
Communication with Resident's Families
We communicate with resident families openly, as long as the resident has indicated we may speak to that particular person. For someone whose Power of Attorney has been activated, we generally only reach out to the person who is designated as their Patient Advocate. We update families when there are changes in their loved ones’ needs or if we need to be discussing care plans. Visitors are welcome at any time.
Short-term or Respite Stays
We offer short-term (respite) stays based on availability of rooms. The cost will vary, based on a person’s level of care.
Pet Policy
We welcome our furry friends as visitors but they cannot be residents unless they are an approved service animal.
About Raybrook Manor & Holland Home
As you may know, Raybrook Manor is part of Holland Home, which has been providing assisted living in the Grand Rapids area since 1892!
The Life Care Promise
Holland Home offers the Life Care Promise, which means that once people are financially approved to come into a Holland Home facility, IF then, at some point, they should run out of funds and cannot fully cover their expenses at Holland Home, we will not ask them to leave.
Entrance Fee and Financial Requirements
Because of this promise of life care, each person coming into Raybrook Manor must pay an entrance fee of $5,000. In addition to the Entrance Fee, each person who wishes to live at Raybrook Manor needs to demonstrate sufficient income and assets to afford at least 3 years of care at the cost of the level of care at which they enter. For example, using the prices given for Level Two Assisted Living (DeVos Center, listed one-third down the fifth page of the packet, left column) in the Single Apartment, Style 3 (dimensions shown on the sixth page of the attachment), a person would need to show they could pay 36 months times $5460/month. We look at the person's monthly income and all forms of assets when we are reviewing financial information. A person pays month-to-month but must show the ability to pay at least 36 months. The “up front” money is the Entrance Fee, which is paid before a person moves into Raybrook Manor.
Assessment for Admission
All applicants are evaluated prior to admission, so that we can determine their level of care. We want to make sure we are providing the right amount of assistance to our residents. When determining a person's level of care, we look at how much help a person is receiving to perform their Activities of Daily Living (ADLs), such as Bed Mobility, Transfer, Locomotion, Dressing, Eating, Toileting, and Hygiene. Each category is scored based on no need; supervision/monitoring/set-up; limited assistance (at this point the care becomes hands-on); moderate assistance, or maximum assistance (at which point the caregiver is doing the whole task for the person). When determining level of care, we also look at any sort of medical complexities/health history which may increase the need for staff assistance.
Skilled Nursing at Raybrook Manor
Raybrook Manor also offers Skilled Nursing for residents whose care needs exceed what can be effectively managed in an assisted living setting.
