Assisted Living Fee Schedule
Heartlands offers a variety of suite sizes to accommodate personal tastes and preferences. Some of our suites are spacious enough to be a home for two people. We offer options and choices to suit your individual needs and are prepared to respond if your needs may change. All monthly rates include the base services and amenities.
OUR ASSISTED LIVING SUITE MONTHLY RATES:
We offer thirteen different floor plan styles and sizes, including one- and two-room suites.
- Individual Suites range from $6,090 to $7,920.
- Companion Suites range from $3,900 to $4,250
Our companion suites are for married couples, partners or siblings.
RESPITE CARE:
Respite care is available for a maximum of 30 days. Prices vary with room size.
MEDICATION MANAGEMENT: (BASIC)
A monthly fee of $640 will be charged for ordering, administering and managing a resident's medications. These services will be performed directly by a licensed nurse.
MEDICATION MANAGEMENT: (COMPLEX)
Includes Diabetes Management for residents requiring blood sugar monitoring and insulin injecting, nebulizer treatments, dressing changes, frequent skin treatment and administration of complex or numerous medications (10 or more routine medications). The fee for this program is $755 per month, which includes medication administration.
THE CARE PLUS PROGRAM:
Each program of care is customized to fit current needs, yet flexible enough to be modified as needs change or increase. Our multi-disciplinary team will work closely with residents and their families to determine the appropriate level of care. Our base service or Care Plus services are available to achieve our shared goals of wellness and independence. Should supplementary care be needed on a regular basis, the Care Plus Program is available for an additional $615, $770, $1,120 or $1,350 per month based on individual needs.
INITIAL SERVICE FEE:
This one-time fee in the amount of $3,000 is paid prior to occupancy. The initial service fee is refundable, on a pro-rated basis, within the first 90 days of occupancy if the resident, with proper notice, wishes to leave Heartlands.

We’ve been taking care of families for a long time, understand care needs and have many delighted families who will share their experiences.

Staff Longevity
Almost all of the management team has worked at Heartlands for at least 6 years, some over 15 years plus. I also have several hands-on staff (nursing, dietary, etc.) who have been with us for 10+ years! We enjoy low staff turnover. Happy employees mean better service and happy residents – Happy residents mean happy families!!
Care Highlights
We have a 24/7 Nurse ON DUTY and ON SITE rehabilitation Program.
Size
Our Assisted Living area has 59 suites and our Bridge to Rediscovery (Alzheimer’s & Dementia) Area only has 9 suites. Our Maximum is 82 residents- small enough to know everyone/ large enough for the bells and whistles!
