place does not look like pictures. my grandfather had a tiny space with a small tv with no remote. the nurse didn't know english well so it was hard to understand her. people were just left in the fall in wheelchairs slumped over. i would never wish any one to stay a day at this place. someone needs to go in there and inspect and make some changes. so sad

I'm so happy to have found and placed my Mom,  Franziska, at Greenfield Care Center of Fillmore, for the following reasons. My first contact was with Mandy and Judy, two amazing professionals who represent the center well. I explained my Moms severe dementia prognosis and the current Skilled nursing facility in Sylmar that was treating my Mom miserably to Mandy and she immediately took action and by the grace of God they had a bed available. I drove out to the center that day and she gave me the grand tour. The facility is very clean inside and out and all the staff are very friendly. I had to laugh, this one gentleman on the tour, Matais, who runs the fun activities programs for the patients was singing karaoke to them in the activity room and would pull passing staff to sing a song and you know what, not one turned down his requests, they all were happy to participate. The next day, I drove my Mom to the center and Mandy placed her. Such a blessing!One of many things so far that has been impressive is the immediate phone call notification when there's an incident or any changes, I can always count on Greenfield to keep me informed. One incident that comes to mind is when my Mom was dizzy and fell. RN Jessica, immediately called to inform me that my Mom was being transported to Santa Paula Hospital's emergency room for treatment and testing, by the way, another great facility in the network. Jessica was awesome and her follow up was top notch. I would recommend Greenfield Care Center of Fillmore to anyone that wants there loved one taken care of. Thank you Greenfield!

My dad stayed here for almost 7 weeks after suffering a moderate stroke at the end of April 2018. The CNAs were all very patient with him, and tolerated his temporary difficult behavior-we are grateful for their help.  He was confused, agitated, and really hated being there, but about 2 weeks before leaving, his memory and mind came back, and he was much better. A psychiatrist who was treating my dad in-absentia prescribed anti-psychotic drugs to calm him, but they had terrible side effects...so I instructed they be stopped on the advice from his new Primary Care Physician not associated with the facility.  Sad to learn this is how they treat a lot of patients who have difficult behavior.The facilities are rather old and in mediocre condition, but the very nice maintenance staff are always busy trying to make improvements. The heating and A/C don't work too well, but you can only do so much on a limited budget. Don't expect the Taj Mahal, but it's acceptable, no doubt. The nursing staff are excellent and very caring (some certainly better than others). There are a few nurses with zero personality, but they do their jobs just fine. Let's face it, it's a tough job to care for people who are mostly in very poor health. I did notice that they all tended to ignore the few people who ask for help constantly (call buttons going off often)...I didn't like seeing that. During the first couple of weeks, the call button in the room next to us was going off very often, and the male patient would yell loudly for nurses in Spanish. I questioned why doesn't someone help this man and was told by the male nurse on duty that the man always asks for help but he doesn't really need it, so we ignore him sometimes.  Wow...that bothered me. The physical and occupational therapists are all really nice and also very caring. The admin staff are all pretty good, but not as sincere as I would have liked considering this very difficult situation we were going through. They didn't even acknowledge me or say hello when I pass them in the halls on a daily basis-they knew who I was. I didn't like or appreciate being lectured once by the Office Manager (slowly repeating stmts multiple times as if I don't understand English), but otherwise they accommodated my (Alzheimer's) mother also moving in for a month, which was a huge help. The message she needed to convey was appropriate but the way she delivered it was not. I had to bite my tongue because it's more important for my dad to get proper care than for me to get something off my chest. The food is pretty bad, sorry to say. I know they have to accommodate different diets, but most of the time, it was dry, overcooked and unpleasant to look at. My dad said it didn't taste good most of the time. Lots of canned fruit and vegetables. If this was a restaurant, they would go out of business, it was not good at all. My dad gained 30 pounds in the first 2 weeks, and he's a small man.  The medication may have increased his appetite, but for being on a low sodium, low fat diet, his very recent blood work results after being there for a month and a half were terrible (high sodium, high potassium), so towards the end of his stay, we started bringing him food from home or finding healthier options elsewhere (eating out). I had mentioned to a few staff that we were eating out because the food was bad, and they all replied, "Yes, I've heard the food is not too good".  Maybe time to make a change in the kitchen...ya think??I think the facility has changed hands sometime during the past couple of years, certainly improving the conditions and care provided as described in old reviews and public ratings/posts on government sites.  It's a 30-minute drive from my home, but bottom-line, I would bring my dad back here if we are ever faced with the same situation, but you have to be prepared to visit everyday because that makes a big difference in the care one receives.  I had visited other facilities and this one is pretty good in comparison to some of the nightmares out there when your elderly parents can only afford Medicare or Medi-cal options.

This place is always too hot or too cold! My grandma has been under their care for awhile now (2015). My family had hope of changes, but to our dismay the owners don't want to spend any money to improve this place.  Management changed briefly earlier this year (2017) and we were hopeful, but again changes and we are not a fan of the current administration and/director of the nurses. Nurses are not helpful when we have questions and appear overworked and unhappy to say the least. When will their be some kind of stability for our lived ones?  Too bad we have such limited choices here in Fillmore.

I placed my brother with a traumatic brain injury in this facility in 2012 and he is still there.  He is happy, meals are good, looks well and he is offered all activities they have and can pick and choose.  I am happy with the care he gets for the most part.  But with any centers you should do a once a month visit to insure your loved one has all the appropriate things they need by checking the room and discussing with staff what the patient is lacking.

This health care facility is terrible. We had my grandpa here w/ Parkinson's disease (shakes a lot). They would make him feed himself so he would basically starve bc the food would fall off the eating utensil before it would get to his mouth. He was supposed to be on one on one care and the day we told them we were going to discharge him soon they stopped doing one on one care. They also lied about the day he got discharged, maybe to try to make more money. THIS PLACE IS THE WORST!!! I do not recommend.

Just went in yesterday to check up on my grandpa who is 95 years old and our experience was terrible. There was no tv is his room and he’s being quarantined. We were able to see him through the window. My grandpa had blood on his pillow because he fell a couple of days ago but when they called us he they said he was okay. We took him food and they had water there but we saw him struggling to reach it and to top it off there was nothing in there. He can’t even serve himself he’s weak the nurses should know that he’s been there for years. I complained in order for them to do their job. We took him a blanket not that long ago and he had rags instead of warm blankets. We will be taking him another blanket and I expect that blanket to be with him at all times. Absolutely wrong, my heart hurts. If you have any loved ones here please check up on them.

This place is a cesspool of toxic leadership and treat their residents and workers with no respect. The nursing staff is unprofessional and lack any degree of competency. Do not ever send your loved ones here unless you just want to see n them deteriorate.This place and all the leadership are a disgrace to healthcare. They are lazy, and will find every possible way to seek the easy way opposed to the right way. Please do your research before you send your loved ones here. Would definitely think twice working here as well.
