Do you guys currently have a waitlist
We do not have a wait list on our Assisted Living; however, we do on our long-term care healthcare side.
Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?
We have a full care team including an Executive Director, Assisted Living Director, Director of Health Services, Assistant Director of Health Services, Social Services, Chef, Plant Operations, and Environmental. Our staff is fully trained to be an expert in their area of operation. We have annual clinical and psychosocial trainings throughout the year to ensure quality of life. Here are the services we offer here at Forest Park:
- Assisted living to which can transition with anyone through all stages in life, we call this aging in place. Whether you are needing a little or a lot of care, we are here to meet those needs.
- Dementia unit opening SOON
- Rehabilitation to help you maintain the most independence! We are the only campus in this area that has specialized training in Parkinson's disease.
- Longer-term care that provides quality of life through activities with stimulation in cognition and physical functioning.
Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?
We ensure safety through staff monitoring and getting to know your needs and wants. We spend a lot of time getting to know our individuals, so we’re able to recognize changes in mood and or overall well-being.
Do you offer any resources or support services to aid residents' transition into the community?
I myself help with transition by visiting often to aid with the transition. We offer support services through social services, we can provide counseling if needed, along with staffing here to help with supporting individuals. Our activities department is dedicated to ensuring our individuals have a great quality of life, with daily activities, including crafts, 2 weekly outings, live entertainment, bible study, 1:1’s as needed and numerous other interactive activities.
Do you offer transportation services for residents?
Yes, we have our own bus while also using community transportation services as needed.
Could you tell me a little about your dining experience?
We have full dining services with a salad bar daily and a variety of substitutions if you do not like the menu item. Fun Fact: OUR CHEF USED TO COOK FOR RONALD REAGAN IN THE WHITE HOUSE. Our food is delicious, I eat lunch here every day and have enjoyed every meal.
What is the process that takes place if a resident has a complaint or an issue they want addressed?
When it comes to complaints, you would tell myself, social services, or our executive director. We will take your concern, document in our system, discuss as a team and the appropriate party will follow up with you with a resolution that both parties agree with.
What is the cost of living in your community?
Our room and board cost 4,859 with an additional LOC. The LOC cost would need to be evaluated prior to admission with an in-person evaluation.
What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?
We update families as needed, whether that be because they’re having an off day, something happened clinically, or for any reason we feel the family needs updated. We schedule care plans quarterly and or as needed as well to ensure everyone is on the same page.
Do you all offer short-term or respite stays? How much does this cost?
We have Respite stays where people can come in and have short-term therapy and we offer respite stays so caregivers can take time to relax. The cost depends on the length of stay and the LOC that is needed. These prices would be established with a personal evaluation.
Are you all a pet-friendly community?
Yes, we are pet-friendly, you can bring an animal to visit or you can have your own animal on our assisted living. This is an extra cost, which varies on if you plan to take care of the animal yourself or if staffing will be responsible to care for the animal. The animal must be up to date on all vaccinations.
Are residents able to get Covid vaccination and flu vaccinations in the community?
Yes
Are residents able to age in place?
Absolutely. We do not have a wait list on our Assisted Living; however, we do on our long-term care healthcare side.
