Fellowship Community Personal Care
Fellowship Community Personal Care offers residents an independent atmosphere with immediate access to support services when needed. Residents, family members and the healthcare team work together with a Certified Registered Nurse Practitioner to create resident-centered care that improves quality of life. Enhancing or maintaining the highest level of independence is achieved through Restorative Nursing Care by specially trained Transitional Restorative Care Aides. Licensed nursing staff is available 24 hours a day to ensure excellence in healthcare.

Ancillary Services
Transitional Restorative Nursing Care
Assessments will continually take place for all residents to help maintain their optimal functional level. This will be provided by a restorative care aide. The cost for this is $15 per 30-minute session. Residents are usually seen by the restorative care aides three to five times per week.
Transportation
Specifically offered for doctor's appointments, there is a charge of $25 per hour for a driver, with an additional $25 per hour for a transport aide, if required. There is a mileage fee of $3 per mile. There is a $50 cancellation fee for transportation cancelled by the resident/resident representative with less than 24-hours' notice.
Personal Laundry
Fellowship laundry technicians will do personal laundry for a charge of $75 per month, per person. Laundry facilities are also available for use by residents free of charge and are located on each floor.
Salon Services "Cuts, Curls & More"
Our on-campus salon offers a wide variety of professional services including Hair Styling & Cuts, Shampoo & Set, Perms, Color, Rinses and Conditioners. A list of services and pricing is available upon request.
Activity Outings
Any resident who participates in an outside activity where funds are required, such as a trip to a restaurant or ice cream parlor with staff members, and does not have the needed funds on the day of the outing will have the expense applied to their bill.

Types of Professionals and Training
We have licensed RNs, LPNs as well as unlicensed staff who receive more than the required DHS education per year. Everyone is CPR certified as well.
Safety and Well-being Measures
Our staff does rounds to check on the residents. The entrance doors are locked after a certain time. We have cameras at all the entrances of the building. Every resident has a RASP and care plan which is made to care for their well-being specifically.
Resources and Support Services
Our medical director is a Gerontologist who assists with the admissions. If the resident does not use our in-house provider and continues to use their community provider, our staff would reach out for a referral if needed. We have Psychiatry services who come to our community, if that was a resource needed.
Dining Experience
Our residents eat in the dining room for all of their meals. They order their meals at the tables and can receive assistance by the staff, if needed. Every meal has a choice of 2 main options. There is an alternative menu, at every table, offered at every meal, if a resident did not want to choose one of the 2 main meals.
Addressing Complaints and Issues
The process would depend on the issue or complaint. We have nursing supervisors on duty who can immediately address certain issues. If the issue needed to be addressed by administration, the staff would notify them. The notification process is explained to the families and residents, at the time of an admission. The phone numbers, of who to contact, are also given to them at that time.
Cost of Living
The cost of living, in our Personal Care, would include the daily room rate and any extra services a resident may request. (Getting their hair done at the beauty salon, buying snacks at our Country Store and charging it to their room, etc.)
Communication with Resident's Families
We have Care Conferences with families. Families can schedule as many as they would want. The VP of Personal Care, Director of Resident Care and the nurse who administers the medications to that specific resident will all be present. The nursing supervisors carry a phone with them 24/7. The families are given that number to call if they have any questions too.
Short-term or Respite Stays
We do have a 30 day or more respite stay agreement. The rate is the same as the daily room rates which were on the brochures. The daily room charge would depend on the room the resident would admit into for the respite stay.
Pet-friendly Community
We are pet-friendly. We only ask for a copy of the pet's current vaccination record. It is a state requirement. The waitlist usually depends on the type of room a resident would admit into.

We have two Personal Care Buildings, the Terrace and Villa. The prices of the different room options are included.
Secured Dementia Care Unit
Our Secured Dementia Care Unit, in Personal Care, is The Courtyard.
