Overview of Professional Staff and Training
We have a nurse as well as a new director that is a nurse. We also have med-techs that are certified and trained or we train them and certify them. All care staff are trained by the nurse and her assistant.
Safety and Well-being Measures
The front door is the only door open for entrance and exiting all day. All other doors are code access entry. Our doors are locked nightly at 7 requiring all visitors to be brought in.
Resident Transition Support Services
We offer PT, OT and other services in our building to residents needing these services. Transitional services (non-medical) is to get involved in activities and the team goes to assist with this.
Transportation Services
We offer van services as well as Catholic Charities transportation services.
Dining Experience
Dining is 8, 12 and 5 and we offer assistance in getting back and forth to meals if needed and in care plan. We offer 10 different meal options each day. A regular planned meal, additional options are available in the anytime menu. Everything is made fresh except for burgers, and chicken tenders.
Resident Complaints and Issues
The resident or family members can visit with the Director or the other department heads about issues. The best way to resolve any issues is to meet with the director.
Cost of Living
Please see the brochure I emailed. It will give you price ranges but you will not know exact until we do the assessment.
Communication with Residents' Families
We email on simple issues and set care plan meeting on health related issues. We call to schedule.
Short-term or Respite Stays
We are currently not offering respite stays.
Pet-Friendly Community
We are pet friendly. Small dogs and cats are welcome. Service pets are an exception on the size of animals.

DO NOT MOVE YOUR LOVED ONE HERE! My mother and I have been for over two years. In this time I have seen many management and staff changes, from what it looks like is poor pay. I have never seen the same staff twice, there’s very few faces that I recognize. I think they’ve gone through three executive directors within the short time that my mother has been here. No friendly faces, just a revolving door of people trying to get paid from your loved ones money. Good luck if you have any issue at all, they will ignore you, they wont help, then when you try to go above the director “Michele Green” the Vice President “Dan” will also ignore your concerns. Tina is the only person that was at “least” pleasant towards me. My fiancé’ died and the hospice team thought it was in MY best interest to tell my mother (dying of dementia) this information - This upset her to no end.. I would have never told her this info, why would they do this?! I had issues with my cell phone and staff has been calling a land line, not a big deal, but when I told them to change the number, I’m assuming they wrote it on a napkin or something and threw it away because it was never updated, if anything was to go wrong they would have a hard time finding me. I updated it TWICE. Poor communication - when communication is everything in this setting. A Christmas tree, other belongings, a tablet were taken and never seen again by staff, staff did not care and said “that happens” I was told to purchase clothing, I actually had her name on a printed label that I sewed on. There was no excuse to lose clothing. Staff would ask me to buy clothes - practically every month. When I was eventually ok’d to move her, her clothes in her closet WERE NOT HER OWN THAT I PURCHASED. I didn’t see anything I bought. I used their very expensive dental service, she had dentures made, not sure why staff wouldn’t let her wear them, but one day when the dental service notified me that she wasn’t wearing them - I asked staff, someone said they put them on their med cart, but then they half way looked around and knew nothing, elite dental doesn’t care to fix this, staff too. She is not made of money!! This is ABUSE. My mom had a stroke and I was never notified! Staff told me she needed to go to carenow one day, Carenow wouldn’t see her and I took her to the ER. This costed $2000 with no insurance and all that happened was she was constipated. This is money wasted on something your terrible staff should have caught. Every single time I went to see my mom I would have to wait “at least” 20 minutes for someone to come to the door at “the cottage” - No one would give me the code to the door, so every time you want to pick up your loved one you will be at the mercy of whoever opens the door at their leisure. When I tried to move my mom after being fed up with her treatment. I told Michelle Green I would be moving her the coming Saturday (after 30 days) but she said I did not give “formal” written warning. My mom did not want to be here nor did I feel comfortable managing her funds to an organization that was just about making money. I tried getting in touch with a lazy executive director for over a month - she didn’t return my call.. ever. I gave 30 days notice - Did not hear back! - so clearly the residents are just a dollar figure. With no calls back and the receptionist (Kimberly) being on my side about her not calling me back - your staff told me - I’m not sure why she’s not calling you back! I was getting avoided! Then when you need paperwork for the new place for a move - they won’t fill it out. The new care manager faxed them and physically delivered it to their team. Nothing in return. I was my mothers only voice. Do not move here, you will regret it. Basically if anything can go wrong here - it will - STAY FAR AWAY! MAKE SURE CIVITAS DOESNT TAKE CARE OF YOUR LOVED ONE How do you sleep at night? On a pile of dying peoples money apparently. Oh and make sure you have HPAA paperwork signed, because after my mom died, I tried to see if medical malpractice was happening … well as soon as I asked for those records they would not give me them stating I didn’t have a HPPA release. Dancing River said “she is no longer in our care and we don’t not have to give you anything”.
