    the nursing staff and the CNA's, also the PT-OT staff, and activities, this is a very old establishment, but with many more sick patients than me, you could always hear laughter from the hallways, or your room, I found that unusual, but great!!!!

    The afternoon I got there they had a dinner tray and dessert waiting for me. My CNA and LVN introduced themselves to me and helped me acclimate into a new routine they made it all so stress free. For a total of 10 days I felt like a winner!

    Awful- i was given this place's name as a possible rehabilitation place for my mom post surgery but all i ran into were cons. First they require a covid vaccination card and will make a copy before being allowed inside but there is no full tour. As soon as I walked in, it smelled of stale cigarette smoke all the way from the smoking area to the lobby/front door. Also it was visibly over crowded without enough workers. They didnt offer a full tour, only a a peek/100 feet in past the smoking porch and not the patient rooms. But from what i could see, there were lights on showing the patients weren't getting care or lacked enough staff. The patient area i could peek was lit up light Christmas lights - always a bad sign. Id go elsewhere.

    We ended up here when our loved one was discharged from Kaiser and was not stable enough to go home safely.  He was weak from laying in the hospital, not using his muscles and needed to regain his strength so that he would not fall at home.  We preferred to take him home, but we understood that some physical therapy would benefit him so we agreed to have him released to this rehab facility.Had we known the kind of care he would have been getting, we never would have agreed to this facility.  When you walk in, the lobby looks clean and well decorated.  It does not smell like some other facilities I have toured and the people were friendly enough, so I had hopes that things would go well.  At first, I was pleasantly surprised when we were getting assurance about how much they would be doing for our loved one and what the physical therapy involved.  I explained what our goals and expectations were and I was told that everything was noted and that everything we requested would be done.  Sadly, this was all misrepresentation Due to COVID, visitation is by appointment only.  30 minute visits are allowed, but they are difficult to get, so we were unable to see for ourselves what was going on.  After a few phone calls with the patient, we quickly learned that nothing we had been promised was happening.  For the first day he was never taken out of bed once.  Supposedly a physical therapy "Evaluation" was done so I assumed that meant they had gotten him up to work with him and see what he was capable of.  I was wrong - the evaluation involved two people watching him eat and having him push against some things to see his resistance.  He was not even taken out of bed for this evaluation.  Since our concern was him being able to get in and out of bed without falling and this was well communicated to the staff and supposedly noted in his chart, I would think working with him to get out of bed would be part of his therapy.  He did finally get a limited amount of therapy for 20 minutes the next day, but it did not work with any of the concerns we had.  We were then told that they have do not do any physical therapy on Sundays.  How does a rehab center not offer rehab 7 days a week?So in three days he was scheduled to have 20 minutes of physical therapy....this program is not going to get the patients stronger.  But I do not think their goal is to help the patients improve.  The goal is to keep them there an bill the insurance.  This did not seem to be a short term rehabilitation center.  We learned later that there were a number of patients that appeared to be there for mental deficiencies that are long term residents.  There was a woman wandering the halls barefoot talking into her remote control like it was a telephone when my sister went by to drop off clothes.I don't think anyone expects the food to be good in these places, but apparently it was so bad and served cold that our loved one requested a protein shake instead of a meal.  They said ok, but never came back with it.  We had to call from the outside more than once  to finally get it delivered hours later.We were originally told that all patients were bathed every other day based on odd/even room numbers.  When I asked for the schedule for the room in question I was then told it was not actually every other day, it is 2 days a week based on room number.  The care takers are also not gentle.  When they do finally get the patients out of bed, they are very rough with them.I am guessing this facility can keep your loves ones alive or they would have lost their license by now.  So if your only goal is to keep someone alive in a place where you don't have to take care of them, then this might work out for you.  But if you have any hope of someone getting healthier or stronger, this is not the place for you.  They will tell you what you want to hear and not follow through.  If you have no alternative and someone you care about gets stuck here make sure to be visible.  Call repeatedly, visit daily, and check in with your loved one constantly to see what is really going on.

    So sorry that we let our dad get transferred there from the hospital for "rehab".  They just let him lay in bed 23.5 hours of the day.  We called several times a day as we were not able to get in to see him for several days after he was admitted.  The staff would tell us what we wanted to hear and made everything sound like it was great.  Then we would talk to dad and get a completely different story.  He said the food was horrible. Without proper nutrition and exercise, he was never going to get better.  After a few days we checked him out against doctor's orders.  We can provide better care at home.

    I was in this place for about 2 months not knowing my employer dropped me from my insurance because I retired early. Soon as Citrus Care found out they more or less asked me to leave. Now I'm at home trying to get rid of an infection I had at the nursing home and my new cheap insured doesn't cover home nursing to keep my huge wound clean. I'm battling this because I had no insurance money going to Citrus Care. So you tell me who or what's more important to them

    Do not put your family member there!!! A one because nothing less! It stinks! The staff ignores the patients!! My sister was transported in with a broken leg from Santa Barbara. After twenty minutes of no staff coming in the Attendants apologized and told her they had to get back up north. She waited and called me in tears I could barely hear her from screaming in the room with her. I didn't know what the do called Kaiser the only Care Coordinator was in the ER, I told her the situation and asked what I could do, she could only help if an ER patient and it was after hours so no one was available. I asked how i could get her to ER she said. 911 so I called 911 and 15 minutes after the police officer arrived the facility called to apologize! Their excuse after over an hour of hot even acknowledging she was there was they had another patient check in!!!  They can only handle one patient at a time, I guess that was the reason for the screaming patients!! I pray their families see this and check on them and get them to somewhere safe! It took us until the next day to rescue my sister!! Two police visits and $183 transport! Do not put your family member there!!!

    Im not going to lie I was a little hesitant to send my grandpa here because of the bad reviews. I ended up being pleasantly surprised. He is a kaiser member and this is one of the facilities they recommended, the admitting process went smooth and he stayed for 3 weeks to get his physical therapy in and came right back home. I honestly don't have anything bad to say about this place. It was clean and organized. I would definitely recommend.

    My grandma is a patient here at citrus and unfortunately it was around the time citrus got covid thankfully my grandma recovered and is feeling a lot better.. the staff was very good with keeping me in the loop.. I knew when she had good days and bad days. My grandma has no complaints the staff did what they could and I couldn't be more grateful.

    Allowed my father to sit unmonitored for a week while his heath detiorated. No one trained enough to chart mental status. No one really cares. Nurse yelled at us for asking her to check on him. Came "highly rated" by va Loma linda staff, but honestly don't see where the 5-star rating came from. They did not inform us of the positive covid patient in the center and my father caught it. Just as he had feared. I wouldn't take my worst enemy to this place. I feel sorry for the few good employees there that were good, but they seem to lack communication between employees.

    I normally don't write reviews but my mom recently was there for 3 days before she past away.. my experience with citrus wasn't a very good one, for starters I was told I could make an appt for a visit for my mom 24 hrs ahead and when I did oh there's nothing available. I was also told I could spend a hr with my mom in the courtyard which they failed to mention it would be on the other side of a gate and plexiglass. Sooo miss leading!!!! For the few days she was in there she told me that during the nights she would try and get a nurse to come in by pushing the button and even yelling and no one came she felt ignored.. which I talked to a day nurse and told them .. last one was the day she passed.. I called early as I work early just to see if she was awake and If I could talk to my mom if she was sleeping then it was ok: the nurse REFUSED to go check on her .. which just over a hr later I was called they couldn't get her to go wake up and took her to the hospital where she passed! Now maybe if they nurse went to check they could have saved her, I don't know maybe it was her time but I'll never know cuz she REFUSED to go in and check on her! Horrible bed side manner! I wouldn't recommend this place! We trust them to take care of our loved ones... RIP mom!

    I cannot even give them one star this is the most horrible horrifying place my brother was taken there on a Wednesday by the following Sunday my brother was in a crapy diaper unable to speak unable to eat unable to walk unable to think a nurse yanked out a sheet from under him it was not going to replace it she said it was for his better I cussed at her I cussed at the head nurse until I demanded they bring an ambulance my brother was taken to the local hospital where they found that he had unenlarged heart his liver enzymes were low his white count low diabetes low and infection his tongue was already having cuts on it from dehydration and I believe he was near death with in 24 hours of being attended at the local hospital my brother is speaking singing and color is coming back I will beg whoever reads this if your loved 1 is there remove them immediately take them out follow up on your loved ones this place is a hellhole God help them on judgment day my brother is doing great he is singing he is alive and I thank God for his grace and his mercy forgiving me the strength to stand up to this evil place.  The only reason I'm tapping one star is because I have 2 other than that they get a negative below 0

    This place is pretty shifty it's so old the closest doors to the patience rooms are barely hanging on the beds have paint coming off its just so out dated and the staff is lazy they didn't feed my mother in law when they were supposed to or give her the medication on time and didn't and as far as getting information over the phone they say that they would call back and never do after a week we rock her out of that he'll whole and she's doing much better my advice to anyone who needs a place to go that you love don't do it BTW the food was terrible.

    Bryce P. THEY ARE HIDDEN in the admission papers unless you've changed your policies since my sisters death! ARBITRATION was NEVER explained! In laymen terms, now that I have an understanding what Arbitration means, nobody in their right mind would sign something and give any nursing home the reins to neglect their loved ones. Its like saying to someone, in this stack of papers is a voluntary agreement to not sue us if we neglect your loved one, but its not mandatory. Seriously Bryce? No one in their right mind would sign it, thats exactly the reason its not explained! Funny thing is, supposedly it was explained to my sister and I (My sister was born with mental retardation) and she completely understood and told me to sign it? Come on, really? As her advocate, I signed admission papers that were marked in yellow and NEVER in her presence and NEVER was it explained, that is flat out fabricated! Im sure that prior to her passing, many others signed just trusting they were signing admission papers only. And you Bryce P? I have seen your replies to other people's reviews in the past identifying yourself as the owner?

    I am a covid 19 survivor. When I came for physical therapy here at Citrus Nursing Center I couldn't get out of bed. Within 2 weeks I have relearned walking and my oxygen requirements have dropped from 6lpm to 3lpm. CNC has an expert staff of trained pros working 24/7 to help achieve independence. The food isn't bad either. I highly recommend this facility for recovery from hospital stays.

    I don’t understand how all these place loose their patients clothes, after family members pay a lot of money for their stuff all of it gets lost then when you see your loved one they look like a homeless bum!!!! These elderly don’t have much and for thier clothes to get stolen or misplaced

    I wish I was able to post photos here, they would certainly turn your stomach! My sister passed away at Citrus in 2018 from repeated neglect. It is worrisome that visitors are kept out during Covid and cant check on their loved ones hygiene and general well being! So many nursing notes were falsified, I didn't discover it till after her death! Please! Please! Be vigilant!

    I don’t care what anyone says about this place because we all have our own opinions. My uncle was here for several years and they treated him like a King. We made and developed many relationships with nursing staff. They were always good to him, answered all our questions during the Covid-19, provided hot meals, very clean and made sure he had his cell phone charged to receive phone calls from us. The nursing staff would notify us right away if anything wrong. My uncle passed away due to old age, and underlying conditions. This convalescent home will always have a special place in my heart for taking care of my loved one who is now with the Lord.

    I've been a resident at citrus for about 5 years now and during my experience I've had a lot of ups and downs but mostly ups. Unfortunately when I first came in the staff wasn't very welcoming.. now that I have warmed up to everyone I've grown to have great friendships with them, staff and residents. Citrus felt like home right away. The people here really care about you. Like any facility you know your good nurses from your bad ones. At citrus I was able to go out In the halls to talk to everyone, I was able to go to the stores, I participated in activities like bingo and bowling and much more. Since covid started, all of that is on pause now. Things are different but I hope it's temporary. The facility itself is very clean and organized. As for the food, it's not going to be the greatest but it is definitely edible! Out of my whole experience and 5 years here I'd have to say I would definitely recommend.. no one's perfect but this is the closest you'd get to it.

    We were referred at this place for my mother in law’s rehab needs prior to Covid and it’s an awesome facility, very clean and inviting. The Administrative staff is excellent and the girls in the front office are very helpful and sweet. Great therapists and nursing staff, very competent and attentive to the patient’s needs.I will definitely recommend this place for any rehab needs.

    I enjoyed my stay at the Citrus Nursing Center for several reasons, which I have stated below. First, I should say that I spent two weeks at Citrus due to suffering a stroke on my left side (I have never had a stroke before). When I arrived, I had no mobility in my leg and arm. This is the placement my health provider provided thus, we did not have chance to research, it was a true learning experience.Positives:Caring Nurses and CNA’s (Certified Nursing Assistants) even during the most difficult timesMany bilingual staffClean basic living and facility areas (beds changed daily)Food is what you would expect, low salt/fat. (I will add, the average CNA had anywhere between 12-18 patients to attend too at any given time. What I found, some of these patients required constant monitoring, thus other patients where left to wait, this also pertains to the nurses. I highly suggest to the management that more staff is hired due to the type of high-need patients Citrus serves)This facility serves both high and low functioning patients. At times, the needs of the low functioning patients had a negative effect on the ability of the staff to attend to the high functioning patients.Inside gray wall paint is very institutional (dark). Wall pictures of old Fontana…why, who cares! All staff wear gray institutional scrubs (yuk). I viewed this atmosphere as counterintuitive to patients wanting to get healthy. Suggestion: a new paint job with positive colors, scrubs for the staff that are bright, cheerful, place slogans of positivity on the walls and hang pictures of famous people who have overcome tragic health/life events. Make sure to educate all staff about the slogans and pictures so they may share and possibly inspire their patients.Final thought: I have read the other reviews, I see their point of view. What I observed and this is from my point of view, families are dropping off family members and thinking that the Citrus staff has a magic-wand that will automatically make their loved-one well. This is not the case, the patient has to want to try and families need to be involved as best as they can (I understand, Covid rules make this difficult). For me, my goal is to be healthy, self-reliant and get back to what I consider normal. The Citrus made a difference in my life… now I’m getting better!Many thanks to Citrus and thanks for reading.

    My mentally and physically handicapped son recently spent two weeks in this facility to get rehab for a broken hip. I was with him daily for several hours. It is not the Hilton, but the best place to recover I have come across in my 82 years. I would highly recommend it. The staff is dedicated and friendly to everybody. The place is light and clean, the meals are appetizing.I am grateful for the good care my son received. As I mentioned before I can highly recommend this nursing home. I do not think there are any better ones. At least I have not seen any when I had to visit several of my elderly friends.

    First impressions are lasting. No one at front desk to start, had to ask administrator for assistance, this facility needs to have better housekeeping, dust on furniture (alot), trash on floors, etc. The only good thing was staff. Very friendly and accommodating. Don't have much confidence in this place.
