MARKET RATE
Comprehensive Monthly Fee: All fees include the costs for Basic Services, the Meal Plan and the Activity Program.

Initial Assessment and Service Plan Implementation Fee:
The first step in determining your eligibility to reside at Christopher Heights is the performance of an initial screening and assessment to determine your service needs and preferences, and the ability of Christopher Heights to meet those needs. Christopher Heights will collect and review information from you and your health providers concerning your medical condition, medication needs, social factors, mobility, activities of daily living (ADL's), instrumental activities of daily living (IADL'S), functional status assessments, social and psychological needs, and other relevant information. Based upon this information, your initial Service Plan will be developed and implemented by Christopher Heights to ensure that your needs as defined in the Service Plan are met. Christopher Heights will charge market rate residents a one-time fee of $5,000.00 to perform these services. Should you not be accepted for residency, you will receive a full refund of this fee. In the event you are accepted for residency, but for any reason decide not to reside at Christopher Heights, then you will receive a refund of $2,500.00 of this fee.
MODIFICATION OF CHARGES
Market Rate Monthly Fee: Christopher Heights reserves the right to change the Market Rate monthly fees in which event a resident will receive at least 30 days written notice of such change in advance of the renewal of the initial term of the Residency Agreement or any extended term thereof.
Please Note: If you believe you are eligible for benefits under the Low Income, Mass Health GAFC or SSI-G programs, please contact our Marketing Representative for information on eligibility requirements and applicable rates.
RATES
LOW INCOME (WITHOUT MASS HEALTH PARTICIPATION)
Currently, to qualify as a low income tenant under applicable HUD Low Income Housing regulations, a resident must have certifiable income of less than $47,760.00 per year for one person or less than $54,540.00 per year for a couple.
Rent:
Studio: $1,190.00
1 Bedroom: $1,275.00
Alcove: $1,190.00
There is no additional rental charge for a second occupant.
Optional Services:
Low income tenants are not required to purchase any optional services from Christopher Heights provided they obtain any services required by their Service Plan from a qualified third party provider. Christopher Heights offers the following Optional Services choices:
Basic Services Package: $3,600.00 for first person and $2,200.00 for a second occupant (includes Basic Services, the Meal Plan and the Activity Program)
Basic Services only: $2,600.00 per person
Meal Plan only: $1,000.00 per person
Activity Program only: $ 300.00 per person
MODIFICATION OF CHARGES
Low Income Rent and Applicable Optional Services: Christopher Heights reserves the right to change its charges for these items in which event a resident will receive at least 30 days written notice of such change in advance of the renewal of the initial term of the Residency Agreement or any extended term thereof.

At the resident's request, Christopher Heights will provide or arrange for the provision of additional services, including but not limited to the following:
- Private companion/attendant care: $20.00 per 1⁄2 hour (subject to advanced scheduling and staffing availability)
- Cable television: $25.00 per month
- Dry cleaning or other specialized cleaning services: Based on services
- Telephone use/service: $25.00 per month
- Barber or beauty shop services: Based on services
- Van/transportation services: Based on services
- Room service: $5.00 per meal
- Sundries, snacks and beverages for personal consumption: Based on purchases
- Additional Housekeeping services: $18.00 per cleaning
- Guest meals: $7.50 per guest
- Medical supplies: Based on purchases
- Referring you to money management services by an independent third party: Based on services
You will be billed for any services provided to you in accordance with the above charges. Christopher Heights reserves the right to suspend the furnishing of any such services in the event you fail to timely pay for them.
MODIFICATION OF CHARGES
Additional Services: Christopher Heights reserves the right to change periodically the amount of its charges for additional services in which event the resident will be provided with a revised Christopher Heights Rate Sheets at least thirty (30) days in advance of the effective date of any such changes.
