THE WORST!!! My Dad, 70 years old, most important person to me, needed rehab for his broken back. This place is beautiful and there are some really nice staff BUT there is not nearly enough staff to accommodate, SO WHY WOULD YOU FILL MORE BEDS IF YOU DON'T HAVE THE STAFF?My Dad chose to ONLY take Tylenol, it would take 2 hours for him to receive Tylenol, and I was in the room witnessing this! Not only that, anytime you pushed the button for someone to come in, there was NO response for anything for at least that long. We did not push the button very much at all, so it wasn't because we were "constantly needy". Bathing was an entirely different story. My Dad went 10 days without being bathed even after multiple requests. When my husband went in to confront, someone had documented he had been bathed when he clearly hadn't. The nurse insinuated that he was not being truthful, so they went down to see my Dad together. Meanwhile, my Dad was crying inside yearning to just be clean! HOW DO YOU EXPECT ANYONE TO GET BETTER WHEN THEY FEEL LIKE THEY ARE DISGUSTING AND GROSS? He did get bathed that day, but not again. It was falsified that he "declined" a shower.Later, reading through his paperwork, he said he was insulted because they had wrote that he was "uncooperative "! Is that them falsifying to cover up the lack of care given? He is the easiest going most compliant person I have ever known, even more so in a medical setting. Or, did they mix up paperwork from a different patient with his? I don't know which is worse!An appointment was set up for my Dad to be transported to the Spine Doctor by The Center Of Arrowhead, they were notified a week before the appointment, my Dad had to miss the appointment because there was NO TRANSPORT SCHEDULED by The Center At Arrowhead. This was them dropping the ball yet again.X-rays were ordered for his back, shockingly, yet again, no one knew a thing or who to get a hold of. NOBODY IS TAKING ACCOUNTABILITY, but this is supposed to be acceptable medical care.My Dad had a wristband that said "Fall Risk", thankfully nothing ever happened but proper precautions were NOT taken by any means. He was left alone in his room with the door closed for hours without anyone ever checking on him. Even when we did press the button for someone to come in, it was hours before anyone ever did. My father could have fallen, hit his head and not have been discovered for hours in a place that he was supposed to be safe and protected, yet was not.The food really super sucked, but at least the food was delivered regularly every day, sometimes late but it was delivered.Physical and Occupational Therapy always showed up and he made impressive progress, but that was only a few minutes a day, as it was ordered. They completely did their job and that was the ONLY time I felt my Dad was safe. Not enough to give this place a decent review in the least.There were a few FANTASTIC people, nurses, therapists, LVN's, that hit us through this experience, but they were not enough to carry the load of this entire establishment. Other nurses who were a bit more serious and fed me a bunch of BS, I don't blame them for being desensitized in this time we are going through and having to work under these conditions that are beyond their control. Puts me back to the F'ing establishment as a whole, the f'ing top dogs of this s*it show, why the hell would you continue to offer beds when you don't have staff to care for them?I could continue on with more of the awful experience, but I will jump to the end. We brought my father home yesterday, they wanted to keep him longer. I would rather shove a pencil through my eye than have him stay another minute there. I feel as though this would be a perfect place to send someone if you didn't care about them and just were waiting for them to die.
If you are reading these reviews, it is likely because you are trying to find a safe, caring environment for your loved one. This is the reason that I am writing this review. The main point of this review is to encourage you NOT to choose this facility for your loved ones. There are many, many issues with this facility, but I will provide just a couple for you to get an accurate picture of the patient experience. My mother arrived there on a Friday night following a hospital stay. On Saturday morning, she informed me that she had been pressing her help button for 30 minutes with no response. I called the front desk to ask someone to go in to help her. The receptionist took my mother's name then placed me on hold. I waited on hold for 12 minutes before being disconnected. When I called back, the receptionist said "Oh, yeah, what was your mom's name again?" By this time, she had been left without assistance for over 45 minutes. Again, I was placed on hold then transferred to a voicemail with a message that said the mailbox was full. It took me about an hour to get anyone to answer, and by then, I was already driving to the 45 minute drive to the facility to assist my mother. When I expressed my serious concerns with my mothers care and informed the facility that I was in contact with Medicare to log a complaint (which was true), my mother's care TEMPORARILY improved. By the following day, we were having the same issues. Once again, I called the facility to request care for my mother. On my first attempt, I was transferred to the on-duty manager "Jason", but got his voicemail and his voicemail box was full so I could not leave a message. When I called back again, I was transferred to the case manager so I left a message. She also did not answer and never returned my call. Many times, nobody answers the main phone number. Given the many issues, our family made a decision to remove her from the facility for another option where she would be safe and properly cared for. My mother arrived at the new facility this evening and I already notice a MAJOR difference.
One star is generous. Let's see the food is not gourmet as advertised, in fact I had to send doordash there atleast 4 times a week so he could eat for the day. Therapists come to the room and sit and play on the phone or watch tv with patient. Other therapist toldmy dad the on the phone therapist is always on the phone.. how about do your job... the first week my dad was there no one outside of the nurses to check on him or food delivery did anyone attempt to come in and work with him. I had to call Brad and ask what wasgoing on, then there were all sorts of people in his room. One night the oxygen in the whole building shut off, instead of going room to room to check on those who take oxygen, ehhh.. my dad wakes up in the middle of the night with oxygen down into the 60s not being able to breathe. They put false information in patients files. When talking to the director, he implies my dad is a liar. Director tells me he's going into talk to my dad, 4 days later we still are waiting. All administration voice mails are full all the time, so you can never reach them. Let talk about the case manager, or lack there of. The one time she actually saw my dad was near the end of the stay when she was to tell him he had to leave. She has no follow through on anything. This past weekend 5/16, they were short on CNAs, the director was called and he never responded to staff...This is a horrible place to put your loved ones. The ONLY 2 good things (which is the only reason they got 1 star) Is Dr. Katz and one of the PT ladies.I will be contacting any and all medical outlets to file a complaint against this place.
Please don't send anyone here. I was a patient for nine days and can honestly say that this was the worst experience of my life. The physical therapy department is good. I also had one caregiver/aide who was totally empathetic and caring. The food was good but the dietary trays were not always correct nor complete. Because I was at the end of a hallway my tray was always last to be served, resulting in lukewarm food. As for the rest; too many errors and more errors. The nursing staff do not talk to each other nor pay attention to anything else going on. I was given medicine no longer prescribed for me. I also supplied my own medicine for part of my stay. I was discharged without any prescriptions for home. I was overmedicated numerous times. One nurse in particular worked at least four double shifts. I asked three times for the soap dispenser at the sink in the bathroom to be refilled after my first two days in the facility and it was finally replaced after I left. The place had an odor, especially at night. The carpet tiles in the hallways are especially filthy and should be replaced in spots. It's too bad that such a nice facility does not live up to its potential.
We brought my Grandma in last night 1-7-2019 she is 96 and unfortunately towards the end. The staff last night was great with the exception of a couple hiccups. The facility was very nice from what i can tell. She arrived at 6:30 p.m. so we didn't get to walk around and really check it out. I will be posting real updates so people can follow our timeline here and make a decision for your family members. Update: The staff were amazing... They were very attentive to my grandma and family. My only complaint would be that they run this large place on a very small crew. I don't think ANYONE can effectively treat Patients like that. I hope management reads this review and hires more staff to ease your current employees and keep them around as well as care for all the people there in the manner they deserve. I felt bad for the staff running their butts off and we were there around the clock for 4 days... Unfortunately my grandma passed. The employees were all wonderful with the process.. Thank you
I wish I would've taken the poor ratings and not trust that they've made improvements to their nursing staff and that they've resolved their issues. First of all, I'd like to thank Jeremy, the Young gentleman from admissions (from Iraq), Kimberly, Nichole, Sandy and Dr. Nassaur for being extra nice. But it takes more than just them to run this place... it lacks professionalism, nursing staff (they mainly have LPNs and CNAs, not RNs), and it always seemed like I was arguing with someone for not providing my mom with the care she needed. My mom came to the facility to get stronger so she can continue the rest of her chemo treatments, but she never got a chance... they couldn't manage her pain, she wasn't getting her meds on time (because they're short staffed). They didn't clean the room/bathroom unless we asked; no bedside manners for most of the nurses; they didn't have time to give my mom showers because they were short staffed. They did not go out of their way to care for my mom... until my mom was put on Hospice; very interesting how the same staff can be super attentive once they get paid more by Medicare once your loved one is on Hospice. Makes me sick to my stomach that they tend to your loved one more once they know they'll pass away and they get more money. A medical records staff approached my 17-yrs old daughter and literally crushed her by telling her that her grandma won't make it to her 50th year anniversary... who the F**K is she to come and do that? What happened to HIPPA... we never asked for her input, but yet she came and told my daughter that she reviewed her records and it doesn't look good. Please, I beg you... DO NOT take your loved one there if you truly love them. Don't let the nice facility fool you! My dearest mom passed away in that facility.
Facilities like these are so invaluable and so are the employees. I recently broke my ankle at work and needed a rehab to learn how to walk again with a boot. I am 52 so the thought of spending time in rehab was a difficult transition from the hospital. What a relief for me. I was welcomed and treated wonderfully. I ended up needing to stay for a month so it became a second home. Anything new and potentially uncomfortable was met with care, understanding and ease. The nurses, aides and PT/OT staff are the best in their field. They take the time to get to know you so can feel at home. Can't forget the kitchen staff, the food was amazing, a pleasant surprise and such a variety. Honestly, it felt like hotel room service as they cater to what you like. You can tell that everyone enjoys caring for people as it shows in how they do their job day in and day out. Clean, nice atmosphere, quick service. They score a perfect 5, every time you press your help button someone is right there!! Shout out to Rhonda, Melissa, Sophia, Stacy, Neal, Melinda and Trish. Thank you for taking such good care of me while I recouped and preparing me to feel comfortable once back at home. I will refer to all my friends and family if we ever need help again.
2 years ago, my Dad was sent here by the Mayo Clinic, after a stay with them. He loved it and he was always happy upon my arrival. Recently, he had a medical emergency, that will require more rehabilitation. He asked if we could set him up there again, "Because they have an excellent Rehab Program there." Well, that settled our dilemma, as to where to send him next. Problem solved. Excellent!!!
My 90 year old grandmother went to this facility. She was neglected. They left her in a chair for over 7 hours. None of the doctors orders were followed. She didn't receive her medicine. When she asked to go to the restroom she was denied several times and told she had a brief on and can go in her brief. Her food was to be puraid and they gave her pancakes and sausage. No one would help her eat or drink. She was not given adequate iv fluids. She left this facility severely dehydrated and almost kidney failure. Dr Shaba met with us and told us all these great things but didn't perform. He said he has no control of people working there. Perhaps he took a different oath when becoming a doctor. Stay clear of this place. Look them up on the Arizona Health Services. Do not send a loved one here. The staff is incompetent. They should not be allowed to be in business!I would give them -5 stars.
I do t know where to begin. My mother is 87 years old with hearing loss, dementia and a broken ankle two places. When searching for a facility to care for my mother I read reviews and came across one that now stands out. The review stated to not let the beautiful facility and smiling faces fool you when selecting a facility. I wish I had listened to that review. I witnessed my mother as well as others waiting extra long times to get assistance to go to the bathroom or come out. I was really annoyed by the attitude of some of the staff and was amazed at how nice Kimberly, front desk and the therapist are. The kitchen staff was so rude to the patients when they didn't think you were around. But as soon as they see you they were overly nice to the patients.My mother was to be well on her way to recovery but do to the nurses and other team members allowed the cast to come off her surgically repaired ankle. Due to the ankle moving freely when the cast was removed allowed the stitched area to open up as a dried out open sore. The screws set in her ankle did not heal as well do to the ankle moving. Due to the negligence on the staffs part my mother was to stay in Arrowhead facility for an additional 25 days and return to the surgeon for review. Well on Wednesday of this week while I was in California I received a call that my mother was being released from the facility in 4 days. I was told she reached a threshold of 75feet for rehab. At 75 feet she is good to go home. I argued the fact that she is in a wheelchair, her bedroom is 15 steps up the stairs and she has a no weight restriction on her broken ankle. My complaints feel on deaf ears of the administrative team. They were so confused on dates of service and release date one said Sunday the other Saturday. They all blamed it on Medicare. My mother used up 50 days would have used another 25 worst case scenario. Medicare will not kick you out of treatment is needed even showing progress. My mothers mind is so confused which was also part of therapy that she had forgotten how to address an envelope.So today I received a call from her nurse that my mother has been released and ready for pickup. My phone was off and I did not see the message until 1 pm two hours after her release from the facility. One employee I need to single out is Ruben not sure what his job is but I am sure he is getting paid an awful lot to be on his cell phone. Each and every time I walked by or up to the nurses station he was on his cell phone. It was annoying to have a loved one in the facility and a staff member was not alert he moved around and answered my questions so slow it was almost in slow motion.As for the food it looked okay but no where near gourmet as the brochure list it as. The rooms are not cleaned enough. I made notice of the same piece of medical package up against the wall for along with crumbs for two weeks.
I was at the facility last summer for one month I am a retired nurse I was so impressed with the staff and the CNA's the care was fantastic food was at best marginal but they did try to accommodate me and meet my wishes. I was very appreciative. I would highly recommend this facility to anyone who needs rehab or even hospice care.
My Dad is 90 and is staying at this facility. It is absolutely gorgeous, so you kind of get caught in the moment. Don't be fooled by aesthetics. It is week 3 for him at the moment. Week one I would have given it 1 star. It starts to grow on you. Medicare has limited choices.You need to be on top of things, or else things can go down quickly. This is true for most of these types of transition facilities. Keep on your toes especially if the patient is elderly. Keep notes and reach out to your case manager to make sure things are taken care of in a timely manner. Make sure you are on top of that. Truth be told in my opinion they need an extra CNA. Each wing has up to 16 patients.1 nurse and 1 CNA for all of these patients is a bit much. I thought they'd disagree, they don't. I asked , they were like "oh this is fine."They are also not on the receiving end. I have timed how long it can take for a response once the call button is pressed. In the day the worst was 11 minutes. In the evening it was 1:30 minutes, that was just once. It was enough for me. Mostly it was close to about 25 minutes. Well, 25 minutes, a person can pee and poop allover themselves. After 5 minutes if there was no response I had to get to the nurses station. I had to make many appearances. It wasn't received well, but when they figured out why I as upset, they got it. What can I say to know me is to love me :-)Changing shifts is also not great. So don't expect an introduction. You have to figure it out once they get into your room. I think this place needs structure, & loose ends need to be tied. That is the most challenging thing here. Annoying...very exhausting. On a good note, the CNA's especially Rosalie , Roberto are great. Most make immediate effort to answer your call. Physical therapy Stacy, & Mark & Roy are awesome. This they have on point. There is an RN on the floor everyday until 6-7pm. Also a weekend RN. An RN is very rare in these types of facilities. Most are LPN's and think they are RN's. I love hearing LPN and RN are the same??? That' s why there are designations. Anyhow there are a handful of good ones; maybe less (3rd floor). LOVE Kelley, my fav one on the floor, Denise is also very good. Marsha is thorough, but may seem not interested. You will find out that she does have the best intentions in mind. Pity I don't have more names. I will evaluate at the end of this stay. Stay tuned...
The CNAs, nurses, and therapists are great at this facility. They are very kind, caring and easy to work with. The place is modern and clean for the most part. HOWEVER, do not expect any help from the administrators. The case managers and main facility staff were awful at following up with patient needs when it came to patient care plans, insurance, coverage, process and procedures, etc. The case managers will rarely return any calls and they make it VERY clear that they are NOT social workers so they are "limited" to what they can assist with. There are no social workers on site, which is very unfortunate for the patients, especially the elderly who are under their care who need guidance. The only one who was very helpful was the kind lady who runs the Medical Records department. Other than that, don't expect to get any sort of assistance from the admin, especially if you're a Medicare patient.
My mother was here in October of 2017. I debated writing a review, and after reading the reviews from the same time frame, I feel like it is even more important to post. I took detailed notes because I noticed things that were upsetting from day one, but rather than detail every story, let me say that what others have experienced is what my family experienced: Nice, new facility with a great PT section. Helpful and attentive PTs.But (and keep in mind that my mother has dementia, so she couldn't even remember that she broke her hip):Concerning lack of knowledge (or lack of concern) on what level of care is required for dementia patients.Negative, apathetic nursing staff. Lack of nurse/CNA to nurse/CNA communication at shift change. Lack of communication with family members.Not enough attention to patients (missed "shower days"/patients getting out of bed without help/reactions to food).No PT schedule except MAYBE a note on your whiteboard some days with a time.Lack of follow up on medical orders like ice packs, wound care, dressing changes, compression socks and ICS breathing exercises.Lack of reaction to things like the possible need for a medication change or doctor review. I too had a meeting with the director to no avail. My brothers and I ended up taking our mom out early to care for her at home.Someone who can fend for themselves and just needs a place to conveniently have PT on a daily basis might do fine here, (I actually talked to some other patients about this) but if you have a loved one who needs attention, help and supervision, you will have to be there yourself 24 hours a day to get that.
If zero stars were an option I would have selected such. Not only does this facility not care about their patients it is very apparent they only care about money. Their director of physical therapy gave my father zero care or physical therapy. Told my paraplegic father he would never be able to regain core strength to do daily activities; ie transfer to and from his wheel chair, get in vehicles without struggle, and will never improve. His 4 week stay would would be cut short at 14 days with minimal physical therapy during with 1 hour physical therapy session for a total of 8 sessions. Valerie Barazza said my father was essentially a lost cause. In a one week span of my father being released from this care facility he has fallen out of his wheelchair twice, one causing several facial lacerations. If you care about your loved ones please do research and send them somewhere else. The only knowledgeable staff they have is their nurses and cna’s. From desk reception will not be able to answer questions about the board, be able to let staff know when you are there for a meeting, the board that you will meet with of care of your loved ones do not care and only care about the money the facility receives.
I was grateful that we chose this place for my MIL. The third floor medical staff took such great care of her and kept us informed at all times when different procedures needed to be performed on her. One of the patients tested positive with Covid so we weren’t allowed to visit for two weeks but we knew she was in good hands. When she was discharged she came home stronger and we could tell that she was healthier! Thank you third floor medical staff for your professional care!
My poor mother in law is in this communication dead zone of a place. Don’t let anyone be admitted here. She presses the call button repeatedly and they ignore it. I call the front desk, have gotten someone to answer 3 times in 25 phone calls. The nurses station has not answered once. The girl at the front desk is very nice and helpful, but good luck getting help w the nursing staff. I’m sure they are understaffed and overworked, but, they should at least answer the call button request. My wife is taking time off of work to be there (The Center) with her as no one else is. Ive lost two parents, and have spent time w both in care centers, this one is beyond what I have seen.DONT BRING A LOVED ONE HERE.

The Center at Arrowhead does a fantastic job providing rehabilitation care. I have worked closely with the Case Management team, specifically with Sherri, who provides the most caring and qualified care to the patients and exceptional support. The new Executive Director has implemented significant changes and is a pleasure to work with as well. I visit numerous facilities in the West Valley, and The Center at Arrowhead is on the top of my list when referring the patients to a state-of-the-art facility with the highest quality of care.

This is my second time here and I can't say enough about how great the care is! The rooms are very nice with your own bathroom. The nurses, and the majority of C.N.A.'s are very good at what they do. The food is very good and portions are sufficient.The physical and occupation therapists are knowledgeable and they have me up and doing things I didn't think I would be able to do.I would recommend this facility to everyone looking for a place to get rehabilitation and care.
My father has visited this facility a few times now since 2018. Facility is nice and clean with a modern look. Second and third floors house the patients during their stay. First floor contains offices to left in the lobby. After signing in at the reception desk you will see the entrance to dining area and down a hallway a physical therapy room. To the right of the entrance dual elevators take you up to visit family or friends in their rooms. Nurse station on each floor will help guide you in the right direction to find the room number. Overall I recommend this facility if you need their services.
Beautiful facility but the guy who runs this place I believe his name is Glenn I’m glad I got his name he is very unprofessional you can here him yelling at his Employees I honestly feel bad for the young lady he was yelling at. Something needs to be done about this no one or an employee should be treated this way. I wish I would have spoken up but I just had to leave a review It happened couple weeks ago.
Was taken there after a week in hospital following spinal surgery this truely was the worst place I've ever been to they put me in a room and getting them to even assist me to the restroom was a challenge the staff has no communication between shifts and couldnt get help from bed to recliner i spent 23 and half hours staring at the wall and only once did they walk me down the hallway needless to say i checked my self out and went home
There is no communication in this facility. Is your loved one being discharged? Better hope someone tells you about transportation. 1 nurse per 16 patients. Waiting for a tylenol for 6 hours. 1 case manager per floor. Patients are filthy and not bathed regularly or thoroughly. Everyone has different information. No communication between staff and staff and families.
