Stole my grandfather's necklace and personal items. Perfectly healthy and then all of a sudden had ulcers and bleeding. Suspicious because this happened after he wanted to go out but they forced him into his room and made him stay there.I would not bring your loved ones there.I went to go check on my grandfather and the nurses were out back smoking. There was another lady in a wheelchair that had soiled herself and they didn't even take care of her.I wish I never had my grandfather brought here. He went from being healthy to having an ulcer when he has never had that issue before and passing away.
My brother Dave has inoperable cancer: this center did everything that they could to meet his medical needs. They went above and beyond in taking care of him: when his needs exceeded their ability, they facilitated and expedited his move to another Avalon facility that has the necessary equipment. Thank you from the bottom of my and my brother David's hearts.
Having worked here about 2 years, I have to say there has been significant improvement in resident care, communication with management and overall appearance change. Staff has a desire to work here as it is like a extended family and there is obviously love shown to our resident. The atmosphere is calm with significant decrease in behavioral outbursts. I would recommend placement here for one of my loved ones.
The facility has made great strides in the past year as evidenced by several measures. The recent annual Utah Department of Health survey was successful with only four tags. The clinical quality measures have improved in the last year and are approaching 5 star levels. The caregivers are friendly, helpful, team oriented, and fun to be around. And renovations to the facility have been made including new painting and are in progress.
This is a wonderful facility! The patient care is great, the communication between family, patient and staff is very impressive. Everyone is very professional and very attentive and nice! Over all, well rounded care facility, highly recommend! Thank you for all you do Canyon Rim!
The most senior member of our family was in the care center for about 2 weeks. He was in hospice care and looked after very well. All personnel were very caring and attentive. His room was simple but always clean. He was visited often several times a day and never found to be need of anything. Based upon our experience I would recommend this care center.
I started work at Canyon Rim January 2017, I joined a team that was mostly new and ready to make Canyon Rim sore again. Our team has grown and with all staff on board have made amazing strides. Canyon Rim is well on it's way back to being a place for healing, caring and love. I enjoy the residents of Canyon Rim, when I walk in thru the doors every morning and see their smiling faces it makes me feel whole again and shows that the staff at Canyon Rim is doing it right. We have come a long way in less than a year and I am excited for the future.
My family has been exceptionally happy with the care my sister has received at Canyon Rim Care Center. With recent changes in personnel, the level of care and coordination of services with family members has reached a more efficient, professional level. For instance, the social worker, Angela, spent more than an hour explaining the specific care my sister received and answering my questions about their programs. Sabrina, who coordinates benefits for patients, also devoted an hour --during a very busy day -- offering advice for my family to enhance my sister's experience at Canyon Rim. Patients are offered individual care to maximize their ability to improve their health and meet personal goals. I appreciate that there are professionals like Angela and Sabrina willing to dedicate their careers to helping others in this important field of caregiving.
Canyon Rim is equipped with a passionate team, that focuses on improving the resident's physical and mental wellness. The team incorporates a network of resources and knowledge into their care. The mission is to be attentive to each residents' individual needs, so that they can work with the resident to maximize their quality of life.This care contributes to the resident's success, not only while they reside in Canyon Rim, but when they discharge into the community. I have seen this first hand, and I am excited to see more improvements throughout the years.
