Overview of Professionals and Training
Our staff includes LVN Nurses and Medical Technicians who receive on-the-job training and sign-offs by a Regional RN Nurse. Our Care Staff undergoes training through the Cares Program, in addition to ongoing monthly training sessions.
Safety and Well-being Measures
To ensure the safety and well-being of our residents, we conduct monthly Inservice sessions for all staff, perform drills monthly, and regularly update our door codes.
Support Services for Residents' Transition
We offer a monthly Family Support Group to aid residents' transition into the community. The Executive Director, Nurse, Care Staff, and I assist in the transition of new residents. The nurse also works closely with the family in the first couple of weeks to ensure a smooth transition.
Transportation Services
We do not have a community van and therefore do not offer transportation services.
Dining Experience
Our dining experience is managed by a chef who prepares meals based on a six-week rotating menu. We also offer an “always menu” for residents who may not prefer the day's meal, such as fish. Mechanical soft or puree diets are also available.
Addressing Complaints and Issues
Our Executive Director is almost always available at the community to address any complaints or issues, providing her contact number to residents and families.
Communication with Residents' Families
We communicate with residents' families in several ways, including updates from our nurse regarding health issues, new medications, falls, and care updates. The Executive Director emails families with updates and newsletters, and I email families about our Family Support Group dates. The Activity Director informs families about special events.
Short-term or Respite Stays
We offer Respite stays at $250 a night and Daystay options from Monday to Friday, 9:30am - 4:00pm, at $150 a day for those wanting to try our community before moving in.
Pet-Friendly Community
We are a pet-friendly community, with the condition that residents must demonstrate their ability to care for their pets. While this is usually not feasible for residents with Dementia, we welcome pets to visit.
Availability and Waitlist
As of today, we have semi-private rooms and 2 medium private rooms available, with no current waitlist.
Vaccinations
Residents can receive Flu vaccines within the community, but we do not offer Covid vaccinations on-site.
Aging in Place
Our community supports residents to age in place, allowing them to live with us through the end of life.
We are a family owned and operated community dedicated to providing the highest level of care for individuals living with memory impairment.
And we would love to help care for your loved one. Our home is thoughtfully designed with the needs of our residents and their families in mind. With a trained and caring staff, a secure community, stimulating activities, healthy dining, and much, much more. Grand Brook provides your family peace of mind.
Our goal is to make each day a meaningful experience for your loved one.
We believe a smile and a hug can go a long way, as loving and personalized care is at the core of all we do.
