The Annandale Healthcare Center is a nursing home that ranks in the top half of facilities based on its overall rating, which is slightly higher than the state average. Despite excelling in certain areas, it grappled with challenges as it received a 2-star health inspection rating, placing it in the bottom 40% of facilities in terms of this aspect, and falling below the state average. However, it compensates for this with a stellar 5-star quality rating, the highest possible mark that a facility can earn. This incredible achievement places Annandale Healthcare Center in the top 20% of facilities when it comes to quality rating, outperforming the state average.Staff Ratings
Annandale Healthcare Center is a nursing home with a 2-star staff rating, one of the lowest possible. It ranks in the bottom 30% of facilities concerning nursing aid staffing hours per resident per day, with a rate of 1.9 hours. This figure is below the state average of 1.94 hours. However, the facility performs better in terms of LPN staffing hours per resident, falling in the top 40% of facilities, although it still underperforms the state average of 1.02 hours with its 0.99 hours. Unfortunately, the Annandale Healthcare Center doesn't fare as well when it comes to RN staffing hours and licensed staffing hours, ranking in the lower 30% and 40% respectively. They offer 0.35 RN staffing hours and 1.35 licensed staffing hours, both lower than the state averages which are at 0.6 and 1.62 respectively. The situation is similar in physical therapist staffing, where the facility is in the bottom 40% with 0.03 hours, compared to the state average of 0.08 hours.Covid Data
The Annandale Healthcare Center is widely recognized for its commendable performance in maintaining resident vaccination rates, ranking in the top 40% amongst all facilities. The resident up-to-date vaccination rate stands at an impressive 72.3%, significantly surpassing the state average of 61.9%, an accomplishment that demonstrates the nursing home's dedication to ensuring the health and safety of its residents. On the contrary, concerning staff vaccination, the center falls starkly behind, sitting at the bottom 10% of facilities. Shockingly, the up-to-date staff vaccination rate barely reaches 0.9%, falling drastically short of the state average of 25.9%. The substantial disparity in percentages between staff and residents vaccination rates indicates an alarming concern that needs immediate attention.Quality Measures
The Annandale Healthcare Center stands as a distinguished nursing home with the highest standard, securing a 5-star quality measure rating. Despite being in the top 40% of facilities where residents experience one or more falls with major injury, it has a lower percentage (2.4%) compared to the state's average of 3.39%. In addition, Annandale is among the top 30% of institutions where residents have received an antianxiety or hypnotic medication, however, it also boasts a lower percentage of 12.04% than the state's average of 19.87%. Despite being positioned in the bottom half of facilities based on the percentage of residents whose ability to move independently worsened, at a percentage of 15.98%, it still maintains lower rates than the statewide average of 18.49%. It's worth mentioning that Annandale is unfortunately among the bottom 30% of facilities in terms of residents losing too much weight, with a slight overstep to 8.09% compared to the state's 6.79%. Despite this statistic, the Annandale Healthcare Center continues to aim for improvements in all sectors of its services.Emergency and Hospitalization Quality Measures

Both of these statistics are considerably lower than the state's average of 1.49 hospitalizations and 1.11 outpatient visits per 1000 days. Moreover, this facility maintains its excellent status with 9.63% of its short-stay residents having outpatient emergency department visits, lower than the state's 13.11%. Furthermore, Annandale is among the top half of facilities when it comes to the percentage of short-stay residents who were rehospitalized after a nursing home admission; with an impressive 22.07%, just below the state's average of 22.09%. Cannot give any star. This place needs to be investigated and closed down. They drugged my sister to death. We were told at 12:15am right after her death we had 4 hours to remove her body or the police would be called and she be deemed an unclaimed body. Horrible administrator and personnel.
Concur with previous reviewer that this place should be shut down! While the staff appeared to be trying, it was clear they did not have the proper training or materials to provide adequate care. The conditions in my mother's room were horrible and I removed her with in days. There were ants crawling everywhere and the patients were given the tiniest bottles of water there are. Absolutely horrible conditions. No working TV and no one to fix it after asking for 3 days, no working phone, no clock or calendar. The sheets were thin, tattered and stained and the room/bathroom were filthy.
This place should be shut down. They were supposed to care for my mother. They did so by letting her sit in her soiled diaper for hours. She would call me every night saying that her calls for help were not answered so she ended up crapping in bed in her diaper. I felt so helpless that I could not go there to help her. She needed assistance to go the bathroom and was told to do it herself. Keep in mind she recently had hip surgery. My mom ending up falling and breaking her femur because she was unattended. Two days after surgery she sadly passed away. I hold this place responsible. The case working gave very little information and in my opinion, didn't know jack shit and couldn't care less. The last month of my mom's life wasted away at this hell hole. The sad irony of it all is my mom was a nurse in a nursing home for 40 years who cared for her patients, unlike these sorry excuse as health workers let alone human beings.
Sleepy Hollow? More like Sleepy Hell Hole. Don't be fooled by the good reviews on the Medicare.gov site.We took our father there after a brief stay at the hospital for him to get intensive physical therapy. When I went there to speak to someone about receiving my dad, I went through the main wing and it reeked of pee. I feel really sorry for the patients there. The other wing (Emerald court?) was a bit better, but still drab and depressing. A TV in the main area was BLARING (movies playing included Scarface and Total Recall). When my dad arrived there, we told the nurses on duty not to serve him pork or eggs in the morning. We arrived to see him again the next morning, we found him in bed with breakfast on the table. Can you guess what was on the plate? Scrambled eggs and Canadian bacon. That mishap notwithstanding, the food looked awful so we had to bring in our own to make sure our dad ate.At one point, his roommate spilled his urinal on the floor. It took housekeeping HOURS to come and clean. I kept spraying Febreeze but the smell still singed my nose hairs. In addition, my dad developed a bedsore while at Sleepy Hellhole and no one bothered to notice. He started having mini-seizures and none of the staff even knew or thought there was anything amiss. If you can avoid it and care about your loved ones, stay away from this godforsaken dump.
I have personally had a friends grandmother stay at this nursing home. She was very well taken care of and the staff was very accommodating. When my friends grandmother passed she was sent a beautiful card signed from all the staff. If that isn’t compassion then I don’t know what is. This place will always hold a special place in our hearts.
Annadelle healthcare center need a full investigation ASAP!!! If I could give them a zero star I would this is not a safe place to leave any of your love ones. I’ve heard tons of horrifying story with neglection not feeding patient. No one is in charge of that place every time you call and ask for someone in charge you could never get a direct line to anyone’s. They keep you on hold and eventually it just lead to an answering machine or them hanging up. I’m really concerned about the safety of all their patient.
I only have kind words to describe the staff and workers. Our loved one received care and the staff was kind and helpful to address any of our concerns. Be familiar with who is caring, know the nurses, and knowing the Dr's, and the special ladies that handle the activities for visitation, is my only advice. Make sure you put effort into being a part of your loved one's experience and don't expect full service! There's always room for improvement with anything when it comes to our loved one's health care. If your loved one is important, make the time and be present..
I pray that GOD step in an reveals the TRUTH. I have someone under their care at this Present moment that fell multiple times. Please do not neglect your Patients they are there Hoping that you will take care of them to the Best of your Ability. These are trying times and it’s bad enough Family and Friends cannot visit due to COVID. Please take care of our Family and Friends we are depending on you. What if it was someone you Love or Care about? GOD watches and sees everything that you do. Please do right by Him by taking care of HIS People with a sincere Heart. Thank you😇🌹🌹❤️💕
My loved one has been in the facility for over 5 years. She's on the locked dementia unit and this facility and staff has taken excellent care of her. They started a Music & Memory program and she sometimes talks in full sentences now! Please don't buy into the bad press-- these are good people. Thank you for taking care of my loved one when I can't.
I've stayed in more than several nursing homes during my long life (89 years -- thanks to my grandbaby for helping me write this). Annandale has been by far the best, but I did stay pre-COVID & I wouldn't hesitate to go back if needed. The staff truly care and made me feel welcomed from day 1. The therapy staff (Nick & Booky!) are all great and very helpful in meeting goals. The dietary department is very flexible and I would call down every day to customize my meal. If you are expecting a hospital this is not it but in a good way. Posting this to support a great facility who helped me get back home and to all the staff who I became close with.
