Established in 1995, Community Home Health Care (CHHC) stepped up to answer the call for increased home-based medical and support services. Our team of expert health care professionals, boasting years of experience and exceptional qualifications, is devoted to setting a new benchmark for excellence in the services we offer.

Community Home Health Care is committed to providing individualized care that caters to the needs of each person. The nurses and PA's from Community serve our house well. We couldn't ask for a more responsible and work-minded main PA and when she was off this last week, although we did have two different fill-ins, they picked up the specifics for each client quickly and were very proficient, friendly and proactive. Thank you for your help!
